Nocardia, Rhodococcus, Gordonia = all variably acid fast, good guesses, but none are the answer in this patient. Actinomyces - is this acid fast? Unless you are referring to actinomycetes as a broader group, Actinomyces genus is NOT acid fast. What else? Respiratory or other.

#IDtwitter 80 y/o F, chronic productive cough, CT w/ bronchiectasis & scattered nodular/ground glass opacities. Multiple BAL/sputum acid fast cultures positive w/ same bug over few months (not TB/NTM). What partially acid-fast organisms can you name (respiratory & otherwise)?

@MizzouID Nocardia? I had one last week been growing for two years, not on treatment per patient decision, however, updated brain imaging and CT chest almost the same..

#IDTwitter how would you approach patients with treated latent tuberculosis after re-exposure? When do you retreat immunocompetent patients?

@IdVilchez Would touch base with the original cardiologist who had recommended the statin. In most of the cases, I have seen statin being held including similar scenarios to what you mentioned.

#IDTwitter in those patients that you decide to treat with Daptomycin, let's imagine an scenario than you can only do Dapto for 6 weeks. What do you do if the patient is taking a stating, let's say 80 mg atorvastatin after Myocardial infarction 2 years ago post 2 stents
