@CMichaelGibson Surely WiFi and mobile workspaces are a small, worthwhile price to pay for primary PCI, TAVI, LVAD, etc. A “Pleasantville”-like view of medicine in the past is reductive and overly simplistic. As an advocate of evidence-based, contemporary medicine, wouldn’t you agree?

@AHajduczok Good thought! There’s an ongoing trial, GARDEN TIMI 78, assessing symptoms and quality of life among otherwise optimzied symptomatic HF patients with a LVEF <50%.

@josecaferca @Obisht But, real talk, did this provide additional info on told of precordial auscultation and a JVP exam? Did this change the course of treatment? This patient would surely have a systolic mirmur consistent with severe AS and JVD with a CV wave on exam

We studied the utility of in-hospital oral diuretic observation in the largest study to date. ahajournals.org/doi/10.1161/CI… We found: 1) the oral diuretic response during in-hospital observation correlated poorly with the outpatient diuretic response. (2/4) @CircHF @YaleHF

@Ricky_Turgeon Absolutely! Now we can Rx dapa in any order in HFrEF, and without EF restriction (so no more looking for coverage loopholes). Also, for those with concomitant diabetes, we can now Rx empa (or semaglutide), with them only needing to be on metformin (or a contraindication) prior.
