Thrilled to announce the publication of our latest open-access research, "Comparative evaluation of analytical methods for CSF proteomics" in Clinical Proteomics! 🍾🔬 @ThomasKislinger @PennStNeurosurg @UofT Read the full paper here: pmc.ncbi.nlm.nih.gov/articles/PMC12… Why it matters: Cerebrospinal Fluid (CSF) is a direct window into the Central Nervous System, holding vital clues (biomarkers) for diseases like Alzheimer’s and Parkinson’s. However, because CSF has a relatively low concentration of proteins, achieving a comprehensive analysis (proteomics) is one of the biggest challenges in translational neuroscience. In our study, we executed a head-to-head evaluation of four distinct methods used to prepare and analyze proteins from CSF: MStern (a robust, membrane-based approach) N-glycoproteomics (focused on a specific type of post-translational modification) Extracellular Vesicle (EV) Isolation (targeting cell-to-cell communication packages) Seer (a novel, high-throughput, nanoparticle-based platform) The Key Insight We showed that the choice of analytical method fundamentally dictates the biological story you can tell. They are not interchangeable! 🤯 To get a complete understanding of a disease, researchers must strategically choose their preparation technique. What We Found Immune Insights: The Extracellular Vesicle (EV) isolation methods successfully enriched proteins tied to critical Immune Responses and defense mechanisms, painting a clear picture of signaling pathways between cells. 🛡️ - our focus on leukemia samples could have confounded this. Protein Regulation: Our N-glycoproteomics workflow provided exclusive access to proteins undergoing Post-translational modification, giving a deeper understanding of how protein function and structure are regulated. ✨ Broad Coverage: For the most general, high-coverage protein identification, the Seer and MStern platforms emerged as highly robust and efficient solutions. 📈 This work provides an essential, data-driven roadmap for neuro-proteomics researchers worldwide, enabling them to select the optimal method to uncover specific disease biomarkers in CSF. 🚨 Exciting study in glioblastoma research! 🧠 Traditionally, resampling brain tumors seemed impossible due to their tricky location—but Dr. E. Antonio Chiocca & team at Mass General just proved it's safe & feasible! Not once, but 90+ samples in 2 patients over a few months. In a groundbreaking multi-institutional trial of CAN-3110 (an oncolytic virus immunotherapy), they analyzed 96 serial samples from 2 recurrent GBM patients using multi-omic tech. Despite MRI showing "pseudoprogression," the data revealed real immune activation & tumor microenvironment shifts—proving these changes are therapy-driven, not fleeting imaging based changed or tumor growth! This opens doors to monitoring treatments in real-time, right in the tumor's native spot while on immunotherapy. The NRG Oncology BN007 trial investigated whether combining immunotherapy (ipilimumab + nivolumab) with radiation could improve outcomes for patients with MGMT-unmethylated glioblastoma, who typically don’t respond well to standard chemotherapy. Results showed no improvement compared to the standard temozolomide treatment—median progression-free survival was actually slightly lower. @MichaelOkun @armaan_km Was there data on longevity of beneficial outcomes?

To shunt or not to shunt, that is the (NPH) question? The first large randomized trial of shunting for normal pressure hydrocephalus (NPH) just dropped in NEJM, and there was clear benefit for walking, and it favored the group receiving shunts. Spoiler alert: Walking improved, however thinking (cognition) and bladder function did not. Key Points: - NPH is a condition usually presenting in older adults where fluid builds up in the brain and it associated w/ trouble walking, memory and thinking challenges as well as bladder incontinence. - Shunting significantly improved walking speed and balance compared w/ placebo. - No meaningful change in cognition or bladder control observed at 3 months. - Risks included subdural bleeding and headaches, underscoring the need for careful follow up. My take: NPH has always been a tricky area for clinicians when trying to decidr 'to shunt or not to shunt.' Here are 5 points that resonated w/ me: 1- Shunting in well selected persons can restore some walking ability in NPH. 2- Thinking did not improve at 3 months. 3- Incontinence showed no clear benefit. 4- Risks to always discuss w/ people considering shunts should include bleeding and headaches (as not uncommon). 5- Walking gains may possibly reduce falls and boost independence, however we must be careful as sometimes improved walking w/o improved balance, can actually trigger falls. nejm.org/doi/full/10.10… @FixelInstitute @ParkinsonDotOrg @alzassociation @SfNtweets @SfNtweets @NEJM
