What makes a good oncologist? 203 Art of Oncology essays. 242 authors. 7 themes. Practicing patient-centered care, cultivating meaningful relationships, communicating skillfully and authentically, exercising emotional intelligence, managing the burden of cancer care, welcoming varied mindsets, and pursuing learning and leadership. Notable disconnect: ACGME wants trainees to independently manage their well-being. Oncologists writing in JCO say good oncologists seek support, create space for grief, and practice self-care communally. Different philosophy of sustainability entirely. And patients and caregivers weren’t the authors here. Any framework for good doctoring in oncology that doesn’t center their voices is incomplete.

Organ preservation in esophageal cancer (e.g., SANO) is promising—but can we trust pCR in adenocarcinoma pts? Our meta-analysis (>7000 pts) found pCR is a weak surrogate for OS in GEA. *immunotherapy trials may be different DOI: 10.1016/j.ejca.2026.116602

Ever heard of negative hyperselection in EGFR therapy? 🚨 EGFRi help in RAS/BRAF WT mCRC, but resistance is common. 🔬 Resistance is driven by HER2/MET amp, PI3K/AKT act, EGFR mut & more. 🧬 Neg hyperselection = refine patient selection for EGFRi 🔗 10.3390/cancers17172804

Presented at #ASCO25: A 3-year structured exercise program after adjuvant chemotherapy for colon cancer improved disease-free and overall survival, physical functioning, and fitness, as compared with health education alone. Full CHALLENGE phase 3 trial results: nej.md/4mh5RPL @ASCO

Presented at #ASCO25: In HER2-positive metastatic gastric cancer, trastuzumab deruxtecan led to longer survival and better response than ramucirumab–paclitaxel. Adverse events were common. Lung toxic effects with T-DXd were mainly low grade. Full DESTINY-Gastric04 trial: nej.md/4k1dwjN @ASCO

#MATTERHORN trial was one of the best studies at #ASCO2025 - addition of durvalumab to SOC preoperatively FLOT was positive for EFS in gastric cancer pts. That study brings #hope and possible increased chances of #cure. Looking forward to seeing the survival data in the future

💊 Can DFS be used as a surrogate for OS in early-stage pancreatic cancer trials? 📈 In a meta-analysis of 29 RCTs (n=6777), DFS strongly correlated with OS (R=0.70 adjuvant | R=0.90 neoadjuvant). 📍 DFS may be a reliable surrogate endpoint for OS. bmjoncology.bmj.com/content/bmjonc…

🧬 MSI-H = hallmark of mismatch repair (MMR) deficiency. 🧵 Prevalence: ~30% endometrial, ~10% CRC, ~8% gastric. But not all MSI-H/dMMR tumors are alike: 1️⃣ MSH2/MSH6 loss → ↑ TMB, better ICI response 2️⃣ PTEN/AKT1 mutations → ICI resistance Checkpoint blockade is now 1L in CRC & endometrial. #MSI #Immunotherapy #Biomarkers 🔗 nature.com/articles/s4157…
