The Andexxa Showpiece A beautiful example of a trial designed and conducted to ensure a drug was able to clear the lowest possible bar to deliver the press releases its manufacturer required. emlitofnote.com/?p=4710

Timely antibiotics and source control are important – but dramatically less critically important unless critically unwell. Observational studies finding associations with time-to-antibiotics and mortality are frequently systematically biased: academic.oup.com/cid/article-ab…

You are in a busy community ED, getting your 🍑 kicked trying to drink from a firehose. You have a patient in septic shock. In addition to fluids, you are giving peripheral vasopressors. Do you *actually* need to start an arterial line, or can you get by with cuff measurements? A prospective observational study published last week of 1852 critically ill adults says yes. There was a mean difference of 6mmHg between the cuff estimated MAP and the A-line. Things that threw off the measurements: -Very low MAPs -Higher BMI -Advanced age journals.sagepub.com/doi/10.1177/08… #emergency #emergencymedicine #foam #foamed #foamcc #army #armymedicine #armyemdoc #meded #icu #criticalcare #shock #resuscitation #research #data #medx #medtwitter

Take note: pub'd online Oct 11 2022, ACC now is formally endorsing deWinter T waves, as well as modified Sgarbossa criteria for LBBB + pacemakers as "STEMI equivalents" and need for immediate angiography. At last! #foamed jacc.org/doi/epdf/10.10…

I am an emergency physician at Parkland Hospital, the busiest emergency department in the country. I have had ZERO physical assaults by patients on any ED healthcare workers in SEVEN YEARS while I was working. It CAN be done and is NOT part of the job. onlinelibrary.wiley.com/doi/full/10.10…
