@dinukawrpt Classical Bartter syndrome patients are polyuric because of decreased urinary concentrating ability which is due to mTAL dysfunction and high prostaglandin production. Tolvaptan won't be helpful and it might theoretically worsen. Tolvaptan itself causes polyuria and polydipsia.

Impressive indeed. Absolutely, very high UPCR is unlikely to b entirely false. In oligoanuric pts, UPCR ~2-6 g/g may cast some doubt about degree of proteinuria and in those cases lipiduria is a very helpful finding to confirm nephrosis. So I’m never quick to trash UPCRs in AKI.

@manasib33 1. Selective glomerular hypofilration (shrunken pore syndrome) 2. Other causes that increase serum cystatin c or decrease creatinine. measuredGFR may be more informative in this case than urinary creatinine clearance.

Eg-3 out of 10 glomeruli show crescents, regardless of how many are globally sclerosed. But it is important to also give the number/percentage of glomeruli that are globally sclerosed so the clinician gets a good idea of the chronicity
