@thomasforth Forgive me if it’s been mentioned already and I just haven’t sent it. IMO, your thesis lacks consideration of socio-historical factors that have prevented this from happening organically. E.g. rivalries like Manchester vs Liverpool and Lancashire vs Yorkshire etc.

England has two potential megacity urban areas. Both about 20 million people. Both 200km across. We've already built the blue circle, centred on London, a very strong economy. We could built the red one if we wanted. Currently it's a very weak economy.

We're pleased to share the second in our annual series of #StateOfMed reports – Workforce. This focuses on the changing shape of the medical workforce across the UK. This year’s report shows that the medical register continues to grow, as does the diversity of the doctors on it. Doctors’ career paths are also changing, with more doctors than ever before choosing locally employed (LE) roles, working outside of formal postgraduate training pathways and with local contracts. In England and Wales, where data is currently available to us, the number of LE doctors grew by 75% between 2019 and 2023 – this makes this the fastest growing cohort on the register. As we look to the next decade, it’s critical that all doctors have the infrastructure, support and resources they need to achieve their ambitions. That’s in the interests of doctors, as well as the interests of a growing and ageing population. I wonder how many are ‘choosing’ to be employed on local contracts? Many possibly are, as training programmes are inherently inflexible. Most are probably victims of training bottlenecks because of arbitrary restrictions on NTN’s. Lots more I could say but nuance isn’t for X!

@gmcuk I suspect that most would rather keep their money and await the outcome of the review. No point paying for regulation if the role is restricted to that of an assistant, is there? Those prescribing rights and ionising radiation requests seem far off right now.

PAs and AAs on the voluntary registers will be invited to apply for registration by the end of January 2025.
