I like to draw parallels between restrictive lung disease and HFPEF. The SV/LVEDV and FEV/FVC ratios look good but hide the fact that physiologically you are living on a lung or heart that does half of what it should.

And not even just consultants. 'This registrar is a bully because they asked questions and expected us to know things'. This isn't about 'pimping' (which can be recognised as a different activity to humiliate to facilitate the senior's ego), but about any attempts to produce knowledgeable and self-educating trainees who don't just work a jobs list without thinking. The problem is that we've become too performatively #BeKind and don't just say 'no, that's not bullying' and this gives many the false impression that they shouldn't have to meet a standard and it's unreasonable to be held to one. We do need to look inwards as a profession as well being outwardly angry and blaming government and systems when trying to understand why we have been so open to replacement with every type of noctor. Unless we insist on applying standards to ourselves, then we don't really have a leg to stand on.

Of course, if you're not actually a trained doctor who has studied medicine, you don't have any of the foundational knowledge and understanding that tells you that you can't just apply a severity criterion from one breathing-y illness to another breathing-y-illness, because on a superficial level doing so 'makes sense'.

@KonokYaroslav @kbalian90 That perhaps both of those takes are extreme and unrealistic interpretations of what is presented as a 'gotcha'. In Israel there are some nasty characters, but that also most people are normal and the police respond as they should.

I disagree with the premise of avoiding describing things by what they are not. Language is limited way to transmit meaning (especially concisely/quickly) and a lot of what is implicit to us as doctors is not to everyone else. There is, correspondingly, a need to use both positive and negative descriptors to help patients and non-medical staff understand a role that is not self-evident to those outside of the medical profession. In that sense I think it is important that they understand that this is a senior doctor but who is explicitly not a consultant - this is really important context for a patient medicolegally and in genera - but I do agree phrasing it like this is unnecessarily diminutive. 'They are senior doctors with extensive clinical experience but are not consultants' would be more reasonable without having to use terminology like 'have not achieved' which sounds pre-loaded with judgement, as if this job role is due to a failure or inadequacy.

There have been some conversations around how ACPs are supposedly safer than PAs, and are better trained, and so can work at ST3 level even if PAs are <F1 level as per Leng. I thought it’d be useful to compare the ACP courses to the PA courses 1/

Counterpoint: Being skilled at a task that only requires practised e.g. manual skill or a procedural understanding of a very limited set of variables with predictable outcomes, you don't necessarily require intelligence to be skilled. Medicine is neither of those things. It is a safety-critical, intrinsically intellectual endeavour. Intelligence is required to be skilled at medicine.
