@sargsyanz Hmm, will be curious to see. Can see it being different combo of things (chronic alcohol use/vitamin deficiency driving mcv up and making normal, inflammation driving ferritin up etc), but happy to learn if more neat diagnosis is present.

Puzzle: My hemoglobin is 13.5 MCV is 90 Ferritin is 100 Iron sat is 23% But I am iron deficient. How can that be? Will post answer tomorrow.

Thinking about the mechanism, I was recalling that stomach acid is also important for liberating dietary B12 for absorption. If his vitamin has B12, maybe it was helpful after all?

@ThisAmerLife Ira poses the question about this being one of the most impactful letters to the editor. What about the one saying Opioids are not addictive!!! Also to the NEJM no less. Seems like letters to the editor cause a ton of trouble when misinterpreted as scientific investigations.
