Deep understanding of complex physiology of #ECLS decapnization pivotal... and keep in mind & early recognize differential CO2 removal 1️⃣ spontaneously breathing patient develops mild tachypnea due to exertion, pain, sepsis... increasing CO2 clearance 2️⃣ decapnized blood from native lungs will reach right BCT, but maybe not all of cerebral circulation due to impaired heart function/retrograde blood flow 3️⃣ this may prompt to SGF reduction resulting in 4️⃣ increasingly hypercapnic retrograde EBF to aortic arch entering cerebral circulation & respiratory drive centers through LCCA 5️⃣ increasingly hypercapnic EBF to brain results in increased RR with further reduction right radial artery PaCO2: decreasing SGF results in vicious cycle, increasing cerebral hypercapnia with paradoxical tachypnea, potentially worsening neurological injury If suspected, measure central/mixed venous & PostML PCO2 (or add left radial A line) to gain data about global CO2 kinetics & support decision on ML/SGF setting; locate watershed zone within aorta by #POCUS + consider/treat underlying causes of tachypnea. @CircAHA 🖇️ bit.ly/4h4zi4F
