Hello medics. I have an interesting trenches scenario for you guys. Let's say you're a medical officer like me in a busy general hospital on call. About 5pm, a lady with obstructed labour was referred to you from the PHC. You discovered the fetal heart rate was falling fast, so you commenced resuscitation, put on oxygen and worked up for cesearean delivery. Because you're in a general hospital, workup took you 2 hours, by which time the baby's heart rate has fallen to the low 90s despite your best efforts. By 7pm, as you were called from the emergency to the theater that everything is ready, another emergency was rushed in. It was a 40 year old barely conscious man with huge lacerations extending from his lower thighs to his right leg and the bleeding is bright red and pulsatile. You assessed the case in your head, it'll take at least 20-30 minutes to resuscitate, find the bleeder and stitch the patient up before referring for expert care. 30 minutes your obstructed baby doesn't have. You're the only doctor in a 40 kilometres radius. The next major hospital is an hour away. What would you do?
