Vitamin C is more than just "immune support" Neurons accumulate vitamin C to approximately 10 mM intracellularly, roughly 200 times the concentration found in plasma. This gradient is maintained by SVCT2, a sodium-dependent transporter expressed almost exclusively in neurons in vivo. The brain is also the last organ to be depleted during deficiency. In guinea pigs (which, like humans, cannot synthesize vitamin C), the brain retained 24% of its vitamin C stores after 14 days of zero intake, while the adrenal glands dropped to 4% and the spleen to 3%. The body prioritizes the brain above everything else. The adrenal glands are the other major site of accumulation. Vitamin C is a required cofactor for two enzymes central to the stress response: 11β-hydroxylase, which catalyzes the final step of cortisol synthesis in the adrenal cortex, and dopamine β-hydroxylase, which converts dopamine to norepinephrine in the adrenal medulla. Padayatty et al. (2007) measured this directly in 26 human patients. After ACTH administration, adrenal vein vitamin C concentration surged from 39 to 162 μmol/L within 2 minutes, while cortisol did not peak until 15 minutes. The adrenals released vitamin C before they released cortisol. This sequence suggests ascorbate must be mobilized for steroidogenesis to proceed. This doesn't mean mega-dosing vitamin C will improve your stress response. Most of this work describes what happens during deficiency or acute demand, not supplementation above adequate intake. But it does reframe what vitamin C actually does in your body: it's not primarily an antioxidant or immune molecule. It's a required manufacturing input for cortisol and catecholamines, concentrated exactly where those hormones are made. Harrison & May, Free Radic Biol Med, 2009. One Brazil nut per day almost doubled thyroid hormone in hemodialysis patient. Thyroid hormone increased from from 27 to 50 ng/dL in the patients, all of whom were found to be selenium deficient. Selenium is required for the conversion of thyroid hormone, and it protects the thyroid gland. A deficiency can lead to hypothyroid symptoms, even if hypothyroidism is not diagnosed. This is because it affects thyroid hormone conversion, which is rarely tested. Selenium calms autoimmune thyroid processes, Hashimoto’s, Graves’, and thyroid eye disease, and it can accelerate the normalization of thyroid hormone levels during extreme stress. “Increasing Se levels via Brazil nut supplementation was associated with improvement in thyroid hormone levels in HD patients, although the amount of Se given was not able to restore T3 to normal levels.” Ref: Effect of selenium supplementation via Brazil nut (Bertholletia excelsa, HBK) on thyroid hormones levels in hemodialysis patients: a pilot study It’s best to check your selenium intake before supplementing using something like cronometer to make sure you aren’t already getting a lot. While many people get very little, some get a lot and an excess of selenium is also to be avoided. It's easy to do with Brazil nuts.

Always keep the SMS you receive after making a UPI or net banking payment. It contains the **transaction/PPBL number**, which is required while filing a complaint. In case of a wrong transaction, call the bank with all details and visit the branch within **3 days** to submit a written complaint. While filling the complaint form, make sure to include the **transaction reference number, date, amount, and the account details where the money was mistakenly sent.**

If a dog bites then next steps: Dog bite. Vaccination status unknown. Treat it as rabies exposure until proven otherwise. 1) Start with the wound. Immediately. •Wash under running water + soap for at least 15 minutes •Do not be gentle. Mechanical washing saves lives •Apply povidone-iodine or chlorhexidine after washing •Avoid tight suturing unless necessary (and preferably delayed) 2) Assess exposure (this decides urgency) Category II – scratches, minor bites without bleeding Category III – bites that break skin, bleeding, saliva on mucosa Unknown dog = assume Category III if any doubt 3) Rabies prophylaxis (do not delay) Rabies vaccine (PEP) Start immediately, day 0. Common schedule: •Day 0, 3, 7, 14 (± day 28 depending on protocol) Do not wait for symptoms. Once symptoms appear, rabies is almost always fatal. Rabies Immunoglobulin (RIG) : for Category III •Infiltrate into and around the wound •Any remaining volume IM at a distant site •Given once, on day 0 This provides immediate passive immunity while vaccine builds response. 4) Tetanus prophylaxis •Give tetanus toxoid if not up to date •Add tetanus immunoglobulin if high-risk wound and uncertain history 5) Antibiotics (dog bites are dirty wounds) Common choice: •Amoxicillin-clavulanate Covers: •Pasteurella •anaerobes •skin flora 6) Observe the dog (if possible) •If the dog remains healthy for 10 days, rabies risk is low •If the dog becomes ill or dies → continue full treatment without hesitation 7) What NOT to do •Do not ignore “small” bites •Do not rely on home remedies •Do not delay vaccine waiting for lab confirmation Bottom line In suspected rabies exposure, time = survival. Wash aggressively. Start vaccine immediately. Add immunoglobulin when indicated. Everything else is secondary

Look closely... a piece of glass One of my followers @Pras_SwiftMojo ordered this dark chocolate from Instamart (Swiggy’s brand NOICE). While he was eating it, he broke a piece to give to his child. The child immediately returned it, saying something had pricked him. To his absolute shock, there was a piece of glass embedded inside the chocolate. Understanding Anemia Through CBC Markers Anemia is a condition where your body doesn't have enough red blood cells to carry oxygen to the tissues. There are many type of Anemia, but the most common ones (deficiency related) are 1. Iron deficiency Anemia 2. Folate/B12 deficiency Anemia ▶️ Hemoglobin: Low Hemoglobin is the hallmark of Anemia. It indicates reduced oxygen carrying capacity Men: Above 13.5 Women: Above 12.5 ▶️ Hematocrit: Hematocrit is the percentage of Red blood cells in the total blood volume. Low Hematocrit can indicate Anemia. Men: 40-50% Women: 38 -45% ▶️ RBC: Low RBC count suggests insufficient red blood cells production or excess loss. Men: Above 4.7 Women: Above 4.2 When all these parameters are on the lower side, it points towards Anemia. To differentiate between the 2 types of Nutrition deficiency based Anemia, you need to reply upon on a few other parameters in the CBC. ▶️ Means corpuscular volume (MCV): It is a parameter that determines the average size of the Red blood cells. If MCV < 80 fL, it suggests Iron deficiency Anemia (& some other conditions). It is also called Microlytic Anemia. If MCV > 100 fL, it suggests Folate/B12 deficiency Anemia (& some other conditions). It's also called Macrolytic Anemia. ▶️ MCH, MCHC: Both of these important parameters in the CBC. MCH is a measure of the average amount of Hemoglobin in each RBC. MCHC is the measure of the average concentration of Hemoglobin in given volume of RBC. When both of these parameters are on the lower side, it points towards Iron deficiency anemia & when on the higher side towards Folate/B12 deficiency anemia. Reading these Markers in conjunction with others like Iron panel, Serum B12, Serum Folate etc will give a proper picture about the nature of Anemia you are suffering from. In many cases, a person may suffer from both Iron & B12 Deficiency & you can spot patterns of both in the CBC. ⚠️ There are many conditions where your Parameters might be off. So self Diagnosis without the right knowledge might not be appropriate.

I was forced to train an intern. Spent 6 months teaching him everything I knew - every system, every client quirk, every shortcut I'd learned over five years. They made him my boss yesterday. Double my salary. Everyone in the conference room stared, waiting for a reaction. I just smiled and congratulated him. The next day, everyone froze when they opened my company-wide email. It said, "Effective immediately, I will no longer be providing training, guidance, or assistance to management. My role description does not include mentoring supervisors." HR called me into a meeting within an hour. My new boss looked panicked - he had no idea how to do half his job without me. Management tried to guilt me, saying I was being unprofessional and hurting the team. But here's the thing. I'd been doing two jobs for years. Fixing everyone's mistakes. Staying late while others went home. And for what? To watch someone I trained get the promotion I deserved? Now my boss keeps showing up at my desk with questions, and I redirect him to HR every single time. The tension is unbearable. My coworkers are divided - some think I'm brave, others say I'm being petty. ------ This is called performance punishment. You’re getting punished for being good at your job. And here is the part that makes it worse. When that promotion time comes around, they don’t promote you because you are too valuable exactly where you are. “We can’t afford to move you. You’re doing the work of three people.” So they promote the mediocre employee who has time to network and play politics because they are not buried under the workload that you created by being competent at your job. So stop being the best performer — start being strategic. Do your job well, but not exceptionally, because exceptional work gets you more work, not more money. And if they want exceptional performance, they can pay exceptional rates. Otherwise, you’re just working yourself into exhaustion while someone else gets your promotion.
