Firstly, I worked in the NHS for 17 years. Secondly, this is not military service. We are not paid while at medical school, so there is absolutely no return of service. There is absolutely no reason to “owe” the NHS anything - as students, we work for free and as residents, we set ourselves on fire to keep others warm, working millions of unpaid hours for salaries that do not reflect our worth. So…nope.

This is happening in every other professional setting. So why don’t doctors’ unions have an answer for the challenges of access and the opportunity of a broadening skills base? Other than, of course, to go on strike and to slag off their colleagues?

Sigh. Hate to spoil the party, but… We really DON'T want a system in which the number of U.S. medical graduates equals the number of residency positions - and the reasons why should be obvious if you think about the practicalities and second-order effects. (🧵)

The plan has always been clear. A two-tier system. It has nothing to do with pay or Resident doctor’s industrial action. The government is happy to pay more to Physician Assistants than the Resident doctors they were supposed to be supporting. No Resident doctors today. No Consultants tomorrow. Simples.
