@rwc90 PAs aren’t the reason surgical trainees aren’t signed off/competent after training. Too many “trainers” aren’t fit for purpose with no incentive to be otherwise. And too many trainees want it handed to them. If you find a good trainer or trainee, hold on to them for dear life.

Many general surgical trainees cannot get the numbers required to be signed off after 6 years of specialist training. Many are having to get finished as consultants. Capacity hasn't increased, so any list going to train a PA is being taken from a resident.

MRI to use as a roadmap for any subsequent surgical intervention required by an appropriate surgeon given location. IR drain alone is suboptimal for me given loculated + underlying nidus for infection remains.

A patient presents 3 months after laparoscopic cholecystectomy in which there were numerous spilled stones. CT shows a phlegmon in a very superior and posterior subphrenic location (see images). The patient is moderately septic. WWYD next? (with permission)
