@TheLastMerlin If the effect hits, it planeshifts WITH the target. Single use. Also it doesn't need to hit the target for the effect to hit. If it hits ANYTHING, including the ground, it's a 50/50 chance that thing is gone. It also planeshifts the wielder is used as a melee weapon

@deathbybadger Had a year long campaign end because of a joke item from 8 months prior. It was a handaxe that had a 50% chance to teleport with the target to a random plane. First roll of the session was a nat 20. Teleport effect succeeded. He was sent to Elysium and got demolished by solars
