In 2025, $3.4B was lost to hacks and exploits, and 63% of losses were linked to access control failures. The average loss per major incident reached $24.5M, with a single breach hitting $1.5B. There are fewer incidents overall, but the impact is significantly higher. In many cases, the root of the problem comes down to how access, custody and operational control are structured. The transaction looked completely routine, but had already been altered before approval. Once signed, the funds were redirected and then quickly distributed across multiple wallets and networks. The attack has been linked by investigators to actors associated with the Lazarus Group. Despite the scale, Bybit stated that all client balances were fully covered and the platform continued operating. The key takeaway is simple.
