Research from @CommissionerDA reveals that 89% of Domestic Homicide Reviews reported prior domestic abuse, showing that vital lessons to prevent future deaths aren't being actioned. The launch of a national oversight mechanism to hold agencies to account is welcome and needed.

Today I publish new research which shows that the vast majority of victims and perpetrators were known to services before a domestic homicide. Any life lost to domestic homicide is a tragic failure by systems. We must do more to protect victims and prevent future deaths. (1/4)
