Lastly the pharmaceutical companies already are highly incentivized to not screw up because they reliable and would lose their reputation and billions of dollars and shareholder value

The FDA takes 10+ years and $1B+ to approve a drug. That means every year a cure sits in regulatory purgatory, thousands of people die waiting for it.
Not to mention the timeframe and cost make it prohibitively expensive for all but a handful of companies, meaning countless drugs and drug companies never get started. Can't even quantify the body count of the FDA, but it's high.
