I teamed up with @Liz_Bayley_Physio to create this progressive rehab programme for Plantar Heel Pain.  We've called it 'LiTHE' (Liz and Tom's Heel Exercises) and it's aimed at active and sporty patients. It's not a recipe though so it's best to adapt it to suit individual needs.

Femoral neck stress #s /injuries are high risk injuries and the outcome depends on early diagnosis. Working as FCPs and in community MSK clinics one does come across runners presenting with anterior hip pain. Here’s a slide with some useful information on it #bonestressinjuries

At uni I was taught to check THREADS in a patient’s past medical history. Recently I’ve adapted this to DEC’S THREADS. So why add the DECS?… Digestion - gastrointestinal complaints can be a sign of Low Energy Availability and digestive diseases can be a feature of inflammatory arthropathy (which can cause joint and tendon pain). Eyes - Uveitis (pain, inflammation and redness of the eye) is associated with Ankylosing Spondylitis. Cancer - a history of cancer is a red flag 🚩 as it increases the possibility that current symptoms may be cancer related. Skin - psoriasis can be accompanied by psoriatic arthritis which often affects joints and tendons. This may be more common in those with psoriatic nail changes. The addition of DECS helps the screening process then we can explore relevant areas in more detail. What do you think? Would you use this in clinic? 🤔

The 80:20 split is good training option for distance runners but it’s not a recipe for everyone! 🏃🏽‍♀️ For the best results work with a running coach to create a plan tailored to the individual and their goals.

Sleep and Nutrition should be given enough priority when managing runners. They affect energy availability and if this runs low it could influence endocrine function which in turn has various detrimental effects of bone turnover, ability to recover from an exercise session, etc.

Documented permission / consent from patient to discuss this ✅✅ 6 weeks bilateral sciatic pain Then progressive loss of initiating urination, testicular numbness / ‘warmth’ Subjective bowel function changes but hard to articulate Urgent MRI - no disc bulge or stenosis to cause CES But tumour involving the cauda equina nerve roots - likely benign Astonishing service from @ClevelandClinic 🙏🙏 - being seen first thing in the morning by neurosurgeon to plan Will likely need full spine MRI with contrast

Gait retraining measures e.g. increasing cadence for Patellofemoral pain can be useful in runners. How about increasing duty factor (DF = stance to flight time ratio)? Here’s some information on it. It has however only been tested on symptomfree population so far!
