The Ottawa ankle rules are a clinical tool to help determine if an individual needs xrays following a foot or ankle injury. An xray is indicated when there is bony tenderness of the medial malleolus and lateral malleolus of the ankle and the base of the 5th metatarsal or navicular of the foot. Xray is also indicated if there is an inability to bear weight for four steps both immediately following the injury and at the time of clinical evaluation. Mishra’s test evaluates for the presence of palmaris longus. In part 1, the examiner begins by passively hyperextending the patient’s metacarpophalangeal joints. While maintaining this position, the patient is then asked to actively flex the wrist against resistance. In part 2, the patient is asked to abduct the thumb against resistance while the wrist is held in slight flexion. These maneuvers accentuate the palmaris longus tendon if it is present. Sensitivity is 100%, specificity is 83 to 89%! The Dorsal Radioulnar Ligament or DRUL is a dorsal component of the Triangular Fibrocartilage Complex that originates from the distal radius and inserts onto the ulnar fovea and ulnar styloid, forming a key stabilizing structure of the distal radioulnar joint. It functions to provide dorsal stability of the DRUJ, particularly resisting volar translation of the ulna during forearm pronation and contributing to rotational control.
