An intuitive way to use ultrasound to evaluate sick patients along the peri-shock to full shock spectrum, based on clinical presentation. I call it, “Think High or Think Low.” I made these cards for the new UCLA EM interns to wear with their ID badges. @UCLAEMRes #POCUS

@CorydonWW Think EKG vs cath lab for potential ischemic chest pain. In terms of volume assessment, assessing the etiology of AKI, and/or the question to give fluids or diurese, it is somewhere in the middle, with high accuracy in most scenarios and high repeatability.

@CorydonWW Of course! Dr. Prager’s points are 100% true. With anything in medicine, there are tests that are highly accurate but costly, time intensive, and/or very technical to perform. On the other end of the spectrum, there are tests that are mostly accurate, fast, and easily repeatable.

@CorydonWW As an educator, I am also always considering my audience. For advanced POCUS, yes, there is more to IVC. For most physicians, though, IVC (and US assessment of JVP) are much more valuable and practical.

We just published this in The Journal of the American College of Cardiology -- Advances (JACC Advances) "State of the Art Review".   It is a REALLY interesting read, using quotes from Thomas Kuhn's book "The Structure of Scientific Revolutions" (the book that coined the word "paradigm" and "paradigm shift").  Here is the first quote: “Scientific revolutions are inaugurated by a growing sense, again often restricted to a narrow subdivision of the scientific community, that an existing paradigm has ceased to function adequately in the exploration of an aspect of nature to which that paradigm itself had previously led the way.” —Thomas Kuhn Full text pdf: pdf.sciencedirectassets.com/781222/1-s2.0-…

@chungk1031 We’ve been using phenobarbital for patients presenting with alcohol withdrawal in the ED for quite some time. Many respond with 1-2 doses with improvement to avoid admission. pubmed.ncbi.nlm.nih.gov/20825805/ However, there’s not much evidence to show superiority of either meds.
