There is a misconception that International Medical Graduates (IMGs) take residency positions from U.S. graduates. The data does not support this. Each year, the United States produces approximately 31,000 MD and DO graduates, while there are about 40,000 first-year residency positions available. This means that even after all U.S. graduates match, thousands of residency positions remain unfilled, and these positions are largely filled by International Medical Graduates. In other words, IMGs do not take positions away from U.S. graduates; rather, they fill the positions that would otherwise go unfilled, particularly in specialties such as Internal Medicine, Family Medicine, Pediatrics, Pathology, and Neurology. Additionally, many IMGs work in rural and underserved areas as part of visa waiver programs such as the J-1 waiver (Conrad 30 program) and other service-based contracts. These programs require physicians to work in areas where there is a shortage of doctors. Because of this, IMGs play a critical role in providing healthcare access to communities that might otherwise have limited or no physician coverage. According to workforce data, approximately one in four physicians in the United States is an International Medical Graduate, and a significant proportion of them serve in primary care and underserved regions. Therefore, IMGs are not replacing U.S. graduates, they are supplementing the physician workforce and helping sustain the American healthcare system, especially in high-need areas. With due respect, this is a clear misinterpretation of data. 𝗜𝗠𝗚𝘀 𝗱𝗶𝗱 𝗡𝗢𝗧 “𝘀𝘁𝗲𝗮𝗹” 𝘀𝗽𝗼𝘁𝘀 𝗳𝗿𝗼𝗺 𝗨.𝗦. 𝗴𝗿𝗮𝗱𝘂𝗮𝘁𝗲𝘀. King Edward’s ~150 matches are not from a single batch. These are spread across 5–6 graduating classes, as many IMGs apply years after graduation. Pakistan is among the top IMG contributors to the U.S., alongside the Caribbean and India. Similar trends exist with graduates from Mexico, the Philippines, and others. Now the reality: Visit community hospitals across NYC, Chicago, Philadelphia, Baltimore, Brookdale, BronxCare, Interfaith, Lincoln, and many more. You will find minimal to no U.S. graduates in specialties like: ✅ Internal Medicine ✅ Family Medicine ✅ Pediatrics These are essential, underserved settings. If IMGs don’t step in, millions of patients lose access to care. As for IMGs in competitive specialties: They are exceptional candidates, MD/PhDs, strong research backgrounds, top scores. They earn those positions on merit. Let’s be clear: This is not about “taking spots.” This is about filling critical gaps in the healthcare system.
