Anaesthesia is not a vascular access service and you cannot get good at “difficult” cannulas by not doing them. Also, paediatricians and oncologists are very good at cannulation: call them instead #tipsfornewdocs Finally, do not do this:

@mstotty88 @Lomesome144812 @ElshadKarbasi Radiologist isn't the key step here. Radiographer is. The scanners are already in the DGHs. And whilst we aren't going to report the scan, any one on call for spine will be able to recognise a big disc that might warrant surgery...

@Lomesome144812 @ElshadKarbasi Sounds like more innovative radiology solutions needed eg regional reporting hub etc. if there is any specialty that can use innovative and network solutions to demand it’s the radiologists on call and their need to not be where the patient is.

Sepsis tea trolley, sepsis lift, sepsis ambulance… all this sepsis yet most DGHs don’t have 7-day (let alone 24 hour) MRI for cauda equina… patient presenting on Friday afternoon might have to wait till Monday for their MRI… potentially debilitating… deeply unserious country

@juliaisobela I guess all those depressed babies respond pretty well to feeding and naps. Probably a fairly straightforward clinic, but I imagine the parents response when you have to section their neonate must be a challenge.

Four of our Oxford Junior Clinical Fellows got offered a National Training Number at the National Neurosurgery Selection this year. If we consider those who did the fellowship last year this number is even larger. Neurosurgery is the second most competitive speciality in the UK
