@ABsteward Agree, unless there is data to show abx treatment or longer courses are better, we have TONS of evidence showing unnecessary abx are harmful not only at a patient level but a societal level.

Why clinicians assume that the default is the longer treatment duration and that longer is better and we have to prove by RCT that shorter is better, maybe we need to challenge this culture ! They have to prove that longer is better,default is shorter until proven otherwise!🤔

If a new RCT is published and concluded that 14d decrease mortality in Pseudomonas BSI, I'll be the first one to recommend this, but why do physicians think the longer is better while retrospective data are opposite ? Imagine giving more Abx harms to your patient!🔼MDR, AE Data🙏

@OBGaYN @BradSpellberg Ancef for everyone. Study during pcn shortage shows it works, can give in all pcn allergies, cheaper and more convenient vs pcn. That vanc dosing is based on a poorly designed study looking at cord blood concentrations of vanc, no babies in any of the arms developed gbs disease

@cylewhite @BradSpellberg ACOG recommends penicillin (or ampicillin acceptable alternative) for GBS and only recommends cefazolin for those with low risk allergies and vanc for those with high risk allergies to PCN. How would suggest it be changed?

@BradSpellberg And when it matters if you are wrong or not. Many times, empiric abx have no effect on outcome, eg-osteo, pji, clinically stable. If the cx comes back resistant, just change abx.

This is why only true experts should be allowed to make these decisions. So they aren't driven by irrational panic. And are instead driven by cold, hard facts. What bugs, cause what infxns, resistant to what abx? Start with that. Then you move to, ok, what abx cover those bugs.
