@patrocity428 @panscan27 @morrisonMSK I hear this a lot. Rationale as to why? I am at an academic center where its usage is prevalent. Why use multiple words such as “chronic microvascular ischemic changes” when you can use one word to imply the same thing.

@RyanHessMD Couldn’t agree more. Would not have gone to medical school without my experience as a ED scribe. Also, it undoubtedly made me a step ahead in both medical and clinical knowledge as well as invaluable insight into the medical system as a whole.

What are other physician’s thoughts on AI scribes?
We are starting to use them in our practice, though I continue to have an in-person scribe.
I worry there may be a societal cost for things like this…

Have any other rads noticed PowerScribe getting progressively worse over the last few months? Voice rec horrible, voice macros not loading, etc? Support from nuance is pathetic as well. Are there other better solutions? Is anyone using @radai Omni? I’ve just had it with PS.

@BradSpellberg We see perinephric stranding incidentally all the time. Often chronic. Not saying there wasn’t in this case but I would never call pyelo based on perinephric stranding alone.

I recently evaluated a pt with diffuse mild abd pain & tenderness, no fever, no flank pain, a chronic foley, & a +UA. I was really torn. Could it be the large stool burden on CT + hx of severe constipation? Then i saw the CT read of perinephric stranding. Ah, admitted for pyelo.

I recently evaluated a pt with a fever of 40.5 C, WBC 15,000, & hypotensive/tachycardic. I was really torn. Could the pt have inflammation of some kind? I wasn't sure. But then, thankfully, I saw that someone had ordered a CRP. And that's when I knew there was inflammation.

picks up nodal metastasis and distant metastatic disease better than isolated ct alone
since the primary tumour is of large size, suspicion of metastasis should be there ..before curative intent pet should be performed
