@AshleyGWinter My sense is that liver inflammation affects this, both from what I've heard from patients and from my own experience when I had LFTs in the 900s from mono. Eventually (many months later) my food preferences reverted to pre-mono desires.

No idea why but being hospitalized changed my food desires. I used to be a sweets person and now I just want to chug wasabi peas.

@matthewherper When the abstract reads like this, you can be pretty certain the journal's reviewers and editors told them they couldn't publish the results claiming benefit on secondary endpoints without making this point absolutely clear.

@wil_da_beast630 Wrong. Very high spend, very little long-term outcomes data. SELECT trial in patients with cvd showed no difference in all-cause mortality at 4 years.

@MaxJordan_N @EricTopol @KaminskiMed Yes, and, if GLP-1's really have that effect on addictive behavior, any of the many RCTs currently being run to show this will quickly stop early for benefit.

There are at least 33+ RCTs of sham vs acupuncture for pain indications and pooled together, they are negative. This is probably a bad sham or who knows? Data all from China. Has anyone seen the raw data? Editors are gullible.
JAMA Internal Medicine@JAMAInternalMed

In patients with chronic sciatica from herniated disk, acupuncture resulted in less pain and better function compared with sham acupuncture at week 4, and these benefits persisted through week 52, this RCT found. ja.ma/4eDXnOl

@rbganatra @AnilMakam @nickmmark @doc_BLocke @DrToddLee I think they must have asked experts what absolute survival advantage at one year they'd consider "important" for continuous O2 to be worth it, and they said "6%". They then looked at expected 1-year mortality and converted that to the HR needed to get 6% absolute difference.

@rbganatra @AnilMakam @nickmmark @doc_BLocke @DrToddLee Nice figure. Two things to consider: 1) The point estimate of 0.99 is the HR most consistent with the data; wide CI, but more O2 may well not be beneficial 2) I suspect many people would want more O2 even if it "only" reduced events by 15% (less than the first chosen threshold)

@dmrind @AnilMakam @nickmmark @doc_BLocke @DrToddLee Thanks as always for your insight, Dr. Rind! For those following along, here's what he means by changing the threshold to achieve "nonsuperiority," and that the 95% CI doesn't exclude a 28% relative reduction in the primary outcome with 24h O2 Anything wrong with the below?

@AnilMakam @rbganatra @nickmmark @doc_BLocke @DrToddLee Don't spend a lot of time worrying/thinking about non-superiority or non-inferiority. Look at the CI around the point estimate and decide if you care. More time on O2 could have believably decreased events by 28%. They had to change the threshold to make this "non-superior".

@tedfrank @ProfHayward @jadler1969 Can you provide a link explaining what a "Soros prosecutor" is? Given the way that name is typically used in the US, it's understandable to be concerned that it's a trope rather than a description.

George Soros is proud of his prosecutors, and has bragged about his work getting them elected in an op-ed he wrote. It a descriptive term with an identifiable ideology. What two-word phrase conveying the concept would you prefer I use? (Disclaimer: my public interest law firm does have a lawsuit pending against the Tides Foundation for, among other things, um, conspiracy.)
