Today I was shouted at by an oncologist over the phone for ‘not remembering there’s a patient on the other end of my slide.’ I was making a courtesy call to let them know the biopsy was not representative. Why is medicine so toxic?

Plasma cells showing nice paranuclear Hoffs. In contrast, paranuclear “hofs” are an area of pale paranuclear cytoplasm corresponding to the Golgi apparatus. Plasma cells produce so much immunoglobulin after antigen stimulation that the Golgi is swollen and easily visible as a hof

4/ 7️⃣ Whether scheme pays is right for you depends on many factors, whether you can pay, your age, when you want to retire, health, & if likely to be over LTA when you retire. For older people close to retirement, and over LTA, it can be a good deal. Less so for younger members

@chantalsellis I suggest that you get yourself a decent medical accountant who can guide you. These issues will sneak up sooner than you think. At least you will be better prepared for it 🤷🏼‍♂️

@goldstone_tony At what point do you need to worry? I’m an 8 pa brand new consultant, how long would you you need to be a consultant before you start to have to pay attention? I don’t understand any of this!

If like me, your second "brown envelope" landed today, its important NOT to ignore it. As nice as it was for our ex-SOS to tell national radio the problem "used to" affect consultants, my 5 figure AA bill for 21/22 tells a different story 👇 Sadly you cant write "its sorted" on

So the second envelope arrived. 26 years in the NHS so far. All that study, hard work, going above and beyond, CEAs, research, teaching….. and a 5 figure pension tax bill as a thank you. Will have to cut hours and find other ways to work now; this has to stop. @goldstone_tony
