@healthapiguy @mickytripathi1 @JoshCMandel is spot-on. Interop should use modern APIs and secure patient consent with assumption of "zero trust". How do #TEFCA's "just trust me" attestation and cumbersome broker network protect patient privacy in RLS, queries, and data transmission? #TEFCA makes patient friendly digital business models relying on low-friction real-time APIs difficult - is that the goal?? @ONC_HealthIT with lots to do here.

Important, and not a bolt-on. I think properly addressing #3 will require a significant rethink of network design -- most straightforwardly, figuring out what it means for a patient to connect to and interact with the networks (without having to put trust in a system where "all apps are known good actors and fully trusted by all the networks"). My litmus test here remains "Can I as an individual write my own software to connect to the networks, without anybody other than me having to trust it?"
