@aakashgupta Incredible drive. Took my family 2 summers ago. My daughters got tired of my wife and I calling out waterfall after waterfall. Would go back in a heartbeat

That road is Route 1 in Iceland. A week driving it costs roughly $2,500 per person. Flights from the US run $500-600 round trip. Gas is $8-9 per gallon. A glacier hike is $125. A night in a decent hotel near Vatnajökull is $160-200. Total tab for two people to spend a week staring at that glacier instead of a monitor: somewhere around $7,000. The median American household earns that in about 18 working days. Sitting in a room. Staring at a screen. The people who actually drive that road on a random Tuesday in March fall into two categories: retirees who stared at screens for 40 years and saved enough to stop, or remote workers who figured out how to stare at a screen from Reykjavik instead of a cubicle in Ohio. Both paths run through the screen. The photo is real. The freedom it represents costs $7,000 and 10 days of PTO. The device you’re reading this complaint on is the same device that books the flight.

the world looks like this and we’re expected to sit in a room for 8 hours a day staring at a screen

@northwoods1980 @AdamMeakins @adamdobson123 I’ve had a 5/1 extrusion before and was told by an experienced PT to avoid deadlifts. I rehabbed by going very light & going up. I’m about 10 years out with no recurrence. To your question, Jefferson Curls off a box with a light KB were helpful for me. Mobility >Strength

Honest question. Is your primary message here that rounded back deadlifts like this are acceptable and OK, or that they’ve actually been shown to be beneficial, causing tissue adaptations over time that lead to resiliency and optimal adaptation versus variations that maintain a neutral or extended spine position? Interesting concept. I’ve never tried these but I’d be nervous to. Both of my severe back injuries came from lifting something awkwardly in a flexed spine position that ended up tearing my annulus. The last time it happened lifting a trailer it took almost 3 years to heal. I’d be wary, but I’m curious to hear a more nuanced discussion on your position here. Thanks

Your spine is meant to move, adapt, and get stronger. Not behave like stick that needs bubble wrap every time you pick up a weight.

@WesRoth The nonsense in this video is laughable. And deeply disturbing At RSNA, I looked at several narrow use AI models. They’re good, but not there yet. In the medicolegal USA, the model creators aren’t prepared to take on interpretive risk.

Remember when everyone said AI was going to completely replace radiologists? Dario Amodei just broke down exactly why that hasn't happened yet and what it means for the future of your career. Instead of wiping out the job, AI just ate the highly technical part (reading the scans), forcing humans to pivot entirely to the "human touch" (patient care). Amodei believes we need to adapt to AI step-by-step as it evolves. However, he didn't sugarcoat the endgame: he admitted it is highly likely that AI will eventually beat us at everything, including physical robotics.

@FiSurgi Agree For things I don’t have a clue about (interior design), it’s a big leg up For things I have moderate knowledge (investing), it’s confirming and offers tidbits For areas of deep knowledge (radiology), hit or miss. Sometimes surprisingly good, but some whiffs, too

@RadRural @northwoods1980 @DrAdamBitterman Radiology, anesthesia, and pathology are pretty good determinants of surgical performance. If you can find an unbiassed surgeon in a similar specialty, they frequently are the best judge, but that’s hard to find. They’re usually local partners and biased.

@CMichaelGibson @HeartOTXHeartMD I rode in a FSD Cybertruck this weekend. Impressive tech. Did most of the driving autonomously. But, it tried turning down a road that was obviously closed to any human with vision. “Road closed” sign about 25 feet away. 3-5 years away — I’m not betting on it

@MaddiWulfeckMD Good point As a PP rad who spends a week/yr at an academic institution and recruits folks from fellowships, there’s a lot more overlap than there once was between academic and PP Also, some 100+ PP groups day-to-day act like academic groups

5⃣ Bottom line? Academic radiology is starting to look a lot like community practice. And has consequences for: 📉 Education quality 📉 Mentorship 📉 Workforce sustainability

@teachplaygrub Final tele reads on x-rays are expensive We’re exploring is whether our ED physicians and APPs would send out less x-rays to teleradiology overnight if they had an AI prelim vs no assistance. We’d still final read the next day of course

I know they say it is a game changer! But it hasn’t been changing the game that much. Is it cheap? No. Does it reliably work without crashing? No. But does it help me with the really hard cases I need help with? Also no. 😂 We will see how this post ages!!

Pro tip for medical procedures: ask your doctor if they can buffer lidocaine with sodium bicarbonate before injecting. It significantly reduces the burning sensation. Learned this from an ER doc in med school and tried it on myself. The difference was dramatic.

I'll be writing tomorrow (day job and family stuff prohibits today) about the brief flashes we saw from the young guys yesterday, and why it's so mystifying why the Chiefs continue to refuse to play some of them. Makes no sense to me.
