I've noticed that most opinions on GLP-1s come from people who have never used them. That's usually how paradigm shifts work.

In the STEP-1 trial, participants lost an average of 14.9% of their body weight. People still talk about GLP-1s like they're a niche experiment. The experiment ended years ago. This is now reality. nejm.org/doi/full/10.10…

@PeptideList retatrutide's effect on liver fat is what really caught my attention that's a big deal for metabolic health beyond just weight loss

Retatrutide keeps looking less like “weight loss only” and more like a broader cardiometabolic drug-class shift. Still early for real-world use: dose, tolerability, discontinuation, and monitoring will matter as much as the headline A1C and weight numbers.
Eli Lilly and Company@EliLillyandCo

Adults with type 2 diabetes on our investigational triple agonist saw significant A1C reductions at 40 weeks in the TRANSCEND-T2D-1 trial. Results presented today at #ADA2026 and published simultaneously in @TheLancet. Read more: go.lilly.com/xszmxh

running this interesting stack for the coming 4-8 weeks or so: morning amantadine 100-200mg royal jelly 5g tadalafil 5mg subq selank 500mcg epitalon 4mg vilon 1mg mexidol 250mg ALCAR 500mg mid-day/ afternoon cerebrolysin 5ml (10 days) agmatine sulfate 0.5g evening guanfacine 2-4mg magnesium l-threonate 144mg at need intranasal insulin 20-60iu (non-modafinil days) modafinil/armodafinil (100mg max) NACET 50-100mg citicoline 250mg excluded supplements here as my current supp stack is weird to heal my broken foot. trying something new here, focused on neuroprotection, dopaminergic and executive function optimization, glutamate mitigation, and systemic cellular longevity - not just acute (or even long-term) efficacy. saving selegiline for the next experiments (the MAO inhibition is too limiting, it would be annoying) not medical advice.

what’s crazy about cerebrolysin was that it had almost the exact dopaminergic refresh effects that i experienced on my 10 day silent retreat: • memories you didn’t even realise you forgot comeback like yesterday • adventurous dreams • 0 inhibitory thoughts by the end

50ml cerebrolysin. over 9 days. administered into my vastus lateralis… 𝙝𝙚𝙧𝙚’𝙨 𝙬𝙝𝙖𝙩 𝙝𝙖𝙥𝙥𝙚𝙣𝙚𝙙 day 1: - nothing. day 2: - nothing. day 3: - sleep inertia lifted completely despite only getting 5-6hrs (increase in recovery per hour) - dream become more vivid day 4: - brainfog significantly improved - later day wakefulness improved - overall increases in energy/focus noted - the mind began to enter a new baseline state….imagine floating inside a bullet train with zero oscillation; the mind is still, yet flying simultaneously. day 5: - same as day 4 day 6: - inhibiting thoughts in social settings drastically decreased - overthinking obliterated almost entirely day 7 - slight improvements in emotional resilience were noted - morning brainfog lifted completely day 8: - same as day 7. all effects amalgamated into new baseline day 9 - everything continues to compound - facial aesthetics seemed more vibrant/debloated i am hard to impress when it comes to trying new pharmaceuticals/research chemicals/peptides but cerebrolysin definitely lived up to expectations. it’s a no-brainer addition to anyone’s longevity protocol. I’ll be running this again in the next 6 months or so. not medical advice.

Mazdutide is so interesting to me. In theory you’d get the same hunger suppression as tirz and higher glucagon activity than Reta. No GIP agonism in maz though Literally know one knows about mazdutide.

The more I look at biotechnology, the more I think the biggest health divide of the future won't be information It'll be access Some people will gain access to increasingly better medicines Others won't because of locations, laws and prices That's a conversation almost nobody wants to have pharma-journal.com/future-of-biot…

🤯 JUST IN: INCREDIBLE PHASE 3 DATA ON RETATRUTIDE PRESENTED AT ADA At ADA this weekend, Eli Lilly ($LLY ) presented new Phase 3 data from TRIUMPH-1 and TRANSCEND-T2D-1. BLUF: 28% (70 pounds) weight loss. Unreal. In TRIUMPH-1, participants receiving 12 mg of retatrutide lost an average of 70.3 pounds (28.3%) over 80 weeks. Nearly half achieved at least 30% weight loss, and 65.3% were no longer classified as obese (BMI <30) by the end of the study. Among participants who started with BMI ≥35 and continued treatment to 104 weeks, average weight loss reached 85 pounds (30.3%), with no evidence that weight loss had fully plateaued. What stood out more was the breadth of effect across obesity-related conditions: Knee osteoarthritis pain improved by 73.1% Obstructive sleep apnea severity improved by 60.6% Triglycerides fell by as much as 41.0% Non-HDL cholesterol declined by 24.2% Systolic blood pressure dropped by 12.3 mmHg Waist circumference decreased by 9.5 inches Meanwhile, in TRANSCEND-T2D-1, retatrutide produced A1C reductions of up to 2.0% from a baseline of 7.9%. 90% of patients achieved A1C <7% 85% achieved A1C ≤6.5% 46% reached A1C <5.7%, which is below the threshold used to define prediabetes Patients lost 36.6 pounds (16.8%) at 40 weeks, and weight loss was still ongoing at study end. Obesity medicine is increasingly becoming outcomes medicine. Historically, we have evaluated obesity drugs primarily on percent weight loss... but payers ultimately care about sleep apnea, osteoarthritis, diabetes, cardiovascular risk, and healthcare costs. The significance of TRIUMPH-1 and TRANSCEND-T2D-1 is that Lilly is beginning to show a coherent story across those endpoints, not just body weight. I expect this to expand coverage dramatically. Eli Lilly is the Nvidia of healthcare.

I always feel guilty when I see these posts where the system works. It because all I think of is the 50 orders that got delayed from June 3rd. Ship 1000 orders, and its the five where we failed that I focus on. But, I will say I love our boxes. They are not expensive, (because I would rather spend on mutli-vial testing), but they are pretty.

@BasedBiohacker amantadine's neuroprotective effects via NMDA antagonism are interesting given the dopamine elevation, could be a useful balance for people who've been pushing cognitive enhancers like modafinil too hard

BasedBiohacker@BasedBiohacker·
amantadine. what the fuck am i looking to get out of this vintage drug? amantadine hits dopamine from three angles simultaneously: > reuptake inhibition (weak) >presynaptic release > NMDA antagonism blocking excitotoxicity that means you're effectively getting elevated dopamine while your neurons are actively protected from damage. and the anticholinergic properties balance acetylcholine/dopamine ratios automatically - for some this would not be a good thing, but given the amount of modafinil and citicoline i've been doing, it's an interesting experiment. the focus should feel extremely "clean" and more, the sigma-1 receptor activation adds neuroplasticity ON TOP of the dopamine effects. your brain rewires to maintain improvements. BUT, because its dopaminergic effects are relatively mild and its NMDA/anticholinergic effects are more prominent, i am fully open to the possibility it could have opposite effects (brain fog, dissociation). 100-200mg daily, zero abuse potential, 60+ years of safety data - but watch out for livedo reticularis (rare but can happen). not medical advice.
