In tobacco harm reduction, comparison isn’t optional, it’s the whole point. Cigarettes cause disease primarily through combustion, which releases thousands of toxic chemicals, while vaping delivers nicotine without burning tobacco and therefore with far fewer harmful byproducts. If you don’t compare the two, you erase the relative risk difference that can help smokers switch to a less harmful alternative. Conflating vaping with smoking misleads the public, discourages smokers from switching, and ultimately risks keeping people on the most dangerous form of nicotine use, combustible tobacco.

@ArielleSelya Back in 2015, I was selected to speak to a govt committee. I was new to vaping, and all I had was my story and my fears. I had a feeling that the room was in shock when I spoke -- they'd never thought about real people: just about kids and Big Tobacco and risks to be avoided.

@DrMakaryFDA Not so: @FDATobacco has not embraced real world evidence. Regulation is conducted in an imaginary, idealised world that bears no relation to the real world in which FDA operates. The result is more smoking and more illicit unregulated trade. It’s an absurd blind spot.

Someone should write a book called “Blind Spots”. @FDATobacco and @DrMakaryFDA continue to ignore real-world evidence in regulating tobacco and nicotine: 1. FDA has created a huge illicit vape market bypassing FDA’s near-insurmountable barriers to entry, but regulates as if this does not exist. 2. FDA’s main impact has been to the benefit of cigarette companies, Chinese vape manufacturers and criminal networks. 3. FDA defines youth vaping as the primary problem, yet vaping has almost eliminated youth smoking - a far greater problem. 4. Older adults who smoke are the main public health and healthcare system challenge and FDA has relentlessly let them down. 5. FDA constantly adds doubt, confusion and misinformation about the risks of smokefree products and nicotine, compared to smoking. Fix that, and much good would follow. 6. The demand for nicotine is based on people wanting its psychoactive effects. It cannot be regulated out of existence, but it can be made much safer. Instead, FDA protects the cigarette trade and promotes organised crime. That’s one gigantic blind spot, @MartyMakary

New research debunks the "gateway theory", which claims that smoke-free products lead people to cigarettes. A peer-reviewed study published in Research in Economics, based on 1,329 Spanish nicotine consumers, shows the exact opposite. Nearly all nicotine users (9 in 10) started with cigarettes, typically between ages 16 and 18. The average age of switching to smoke-free alternatives such as vapes or heated tobacco was 27, nearly a decade later. Dual users smoked 34% fewer cigarettes annually and achieved a 42% reduction over five years. Nearly half said they switched specifically to cut down or quit. ❗ Nearly one in five quit nicotine entirely. This shows that the “gateway” runs in one direction, and it leads AWAY from cigarettes. Today, I received an email from another GP who was referred to me by one of the "converts" regarding vaping. This is exactly the outcome we were hoping for—by presenting the facts & scientific studies directly. Private doctors seem more open to discussion regarding THR. 1)3
