The #privateequity #healthcare playbook: Step 1: Woo groups with promises of efficiency, innovation, cash injection, growth, and the negotiating power of consolidation (against insurers, etc.) Step 2: Laden group with massive, expensive debt as part of the “cash injection” to “grow” Step 3: Start the 3-5 year race to the bottom to the next private equity sale or flip. Stick the group with even more debt as you “grow” them through bad acquisitions and expensive executives. Slash core operations and “costs” (ahem, physician workforce…) in the name of streamlining “efficiency”. Overbill patients as a revenue model. Don’t care about anything but P&L’s and valuations. Step 4: Realize you’re hemorrhaging and the group will never recover from the debt burden. Try to merge or be acquired early. When no one will fall for it, DNR the group, and exit ASAP with bankruptcy protections or just exit, leaving doctors, patients, and hospitals scrambling. This is playing out now with Envision and APP — but it’s not just emergency medicine. We (EM) are living it now, but Radiology, Anesthesiology, Pathology — the same is coming…

My son battled in second ever tennis match against a better more experienced opponent. Lost first set. Battled back from 1-4 to win the second in a tie break. In the super tiebreak, was down 8-9 and hit a clear ace to tie. Kid called it out. Won the point and match.

The math: Year over year, the $/RVU conversion rate set by CMS has decreased. We are set to be almost 10% less since 2020 with CMS’ most recently announced proposal for 2024. With the ~17% inflation since then, this puts us at total 23% decrease.😡 in2013dollars.com/us/inflation/2…

Let’s not forget, the @cmsgov $/RVU conversion has decreased reimbursement universally every year since 2020. 6% cut in reimbursement through a pandemic…in an economic setting of 4.5% inflation.
