Mientras el mundo debate cómo absorber el shock del petróleo por la guerra en Medio Oriente, Santiago ya tiene 4.400 buses que no sienten el alza. El 68% de la flota RED Movilidad es eléctrica. Eso son 352 millones de kilómetros anuales completamente desindexados del precio del crudo. A 1,8 km/litro de rendimiento real en urbano y 80.000 km/año por bus, la flota eléctrica evita 195 millones de litros de diésel al año. Al precio actual, con el shock de Irán, eso equivale a más de US$200 millones anuales que no se incorporan al precio del petróleo. Más de US$500.000 al día. Pero el número puntual no es lo más importante. Lo más importante es la estructura del costo. Los contratos de buses diésel tienen indexación al precio del combustible. Cuando el petróleo sube 34% como esta semana, el subsidio estatal al sistema sube automáticamente. Los buses eléctricos no. Su costo energético está anclado en contratos de largo plazo con generadoras de energía renovable. El shock de Irán no les llega. Chile tomó esta decisión en 2017, antes de que nadie supiera que el Estrecho de Hormuz iba a estar en jaque. Lo hizo por calidad de aire y equidad territorial. Pero resultó ser también la mejor política de seguridad energética del transporte público que pudo haber tomado. Continuó porque hacía sentido para los que siguieron. Piñera se mandó el salto, y luego Boric lo aceleró. Hoy, cuando el gobierno debate si puede sostener el MEPCO con el alza del petróleo, los 4.400 e-buses de Santiago son un escudo fiscal estructural e invisible. Cada bus diésel que se reemplaza por eléctrico es exposición permanentemente eliminada al precio del crudo. La próxima vez que alguien pregunte si la electromovilidad en el transporte público vale la pena, muéstrenle esta semana. Then she stopped responding. I figured she moved on. Made new friends. Forgot about our inside jokes. Kept sending memes anyway. Once a week. No responses. Just kept going. Felt stupid but couldn’t stop. She’d been my work best friend. Only person who got my humor. Six months of silence. Then one day. “I’m so sorry I haven’t replied. I’ve been in treatment. Depression got really bad after I left that job. Your memes were the only thing that made me smile some days. I’d read them in group therapy. Saved every single one. Administración de vasopresores por vía periférica, pónganse vergas: Revisión sistemática con metanálisis, >30,000 pacientes incluidos -BAJA incidencia de eventos adversos menores (dolor, edema, extravasación, infiltración): 2.3% -Eventos Adversos Mayores (Trombosis, necrosis), incidencia de 1.4%, pero, sorpresa: Cuando hubo trombosis del vaso fue porque usaron un Catéter Central de Inserción Periférica a la Línea Media, particularmente, y fueron solo 3.2% de los casos. Opening that email a year ago and feeling my heart sink into the pit of my stomach was the worst feeling ever, compounded by the demoralizing process of having to go immediately compose myself enough to go through the SOAP. And, it was made even worse by the fact that, though I love my TY program and the growth I have had this year, I ultimately did not get an EM position in the SOAP, which was what I really wanted. But despite wanting to give up, I kept my nose to the grindstone and I persevered, working relentlessly to overcome the failure of my 2025 Match. Hard work pays off. Pardon the French, but I fucking did it y’all. I am quite literally shaking and crying in clinic. It was all worth it. Every single moment of effort was worth it. I am good enough, and I deserve to be an emergency medicine physician, which is the farthest thing imaginable from how I felt this time last year. 😭😭😭😭😭

vibecoder asks claude code to build a chat app, gets a working prototype in 20 minutes, immediately tweets "just killed slack and discord"… brother you don't even know what a distributed system is. you don't know what database replication means. you have no idea how websocket connections behave at scale or what happens when 50k people are online at once and someone's message needs to show up in 200ms across 3 continents slack has engineers making $300k+ who have spent a decade solving problems you don't even know exist yet. race conditions, eventual consistency, message ordering, presence systems, file storage at scale, search indexing across billions of messages your app works on localhost with 2 connections. that's not the same thing as "killing slack" that's a college homework assignment the prototype is maybe 0.5% of what makes these products actually work in production. the remaining 99.5% is infrastructure, reliability, edge cases, and years of iteration on problems that only surface when real humans use your thing at scale and the worst part is the confidence. "yeah its not perfect but ai one-shotted it, just need to adjust a few things and deploy" - the few things you need to adjust IS the entire product. thats like pouring a foundation and saying you basically built a skyscraper, just need to adjust a few things ai is genuinely incredible for building tools and prototypes. i use it every day. but there's this weird thing happening where people who have never shipped anything to real users at scale now think the hard part of software is writing the first 200 lines of code it never was bro

The “Ozempic body” insult is just aesthetic snobbery dressed up as health advice. When someone loses 20–30% of their body weight, blood pressure drops, sleep apnea improves, joints hurt less, and their risk of heart attack falls. If that offends an influencer’s sense of beauty, medicine will somehow survive. And the irony is thick: Saladino rails against “pharmaceutical shortcuts” while selling a cabinet full of supplements that have far less evidence than GLP-1 therapy. Yes — eat real food. Yes — move your body. I recommend the Mediterranean pattern all the time. But pretending obesity is solved by telling people to “eat like your great-grandmother” ignores genetics, metabolism, medications, environment, and decades of clinical research. GLP-1 drugs aren’t a shortcut. They’re the first effective medical treatment for obesity we’ve had in a century.

"Ozempic bod" is not attractive or healthy. GLP-1s are a shortcut to nowhere good. Eat like a human (single ingredient foods and foods with ingredients your great grandmother would recognize) and move like a human. This is the way.

So as I see patients, I put Claude to work. Problem is, I have too many ideas, not enough time, and could likely use someone who knows what they are doing to actually bring things to the market!
