“माझा कोलेस्टेरॉल नॉर्मल आहे… तरी Heart Attack 🫀 का आला ?” 1. ‘नॉर्मल’ रिपोर्ट म्हणजे सुरक्षित नाही. कोलेस्टेरॉलचा रिपोर्ट बरा दिसतो, पण Small Dense LDL particles वाढलेले असतील, तर धोका तसाच असतो. हे कण जास्त ‘sticky’ असतात आणि आर्टरी ब्लॉक करतात. 📚 Ref: Krauss RM, Circulation, 1995 2. शरीरातली ‘Inflammation / दाह / जळजळ ’- Silent Killer! CRP (C‑Reactive Protein), Homocysteine यांसारख्या चाचण्या जळजळीचं पातळी दाखवतात. रिपोर्ट नीट असला तरी शरीर ‘on fire’ असू शकतं. 📚 Ref: Ridker PM et al., NEJM, 1997 3. “साखर 🩸 नियंत्रणात नाही , चूक फक्त औषधांची नाही तर चाचणीही असू शकते!” २५ वर्षांच्या अनुभवातून सांगतो Glucometer बरोबर Lab test ही औषधांइतकीच महत्त्वाची उपचार पायरी आहे. पण diabetic रुग्ण आणि नातेवाईक तिथेच सगळ्यात जास्त चुका करतात. Diabetes रुग्ण करत असलेल्या 5 सर्वात common चुका: 1 फास्टिंग म्हणजे ‘रात्री काहीही न खाणं’ एवढंच नाही! 8 - 12 तास फक्त पाणी वगळता काहीही न घेता राहिलं पाहिजे. चहा, साखर, दूध घेतल्यास glucose व cholesterol दोन्ही readings चुकतात. Lipid profile आणि fasting sugar योग्य फास्टिंगशिवाय अर्थहीन ठरतात. 2 Post‑Meal (जेवणानंतर) नमुना म्हणजे नेमकं कधी? Post‑Lunch किंवा Post‑Dinner sugar test म्हणजे जेवणानंतर नेमके 2 तासांनी दिलेला नमुना. काही मिनिटांचा फरकही चुकीचा impression देतो , डॉक्टर treatment वाढवू किंवा बदलू शकतात. “थोडं उशीर झालं, काही नाही” असं म्हणणं म्हणजे reports बिघडवणं! 3 औषधं अचानक थांबवणं ही सर्वात मोठी चूक. काही लोक सांगतात, “टेस्ट आहे म्हणून औषध घेतलं नाही.” हे धोकादायक आहे , sugars वाढतात, kidneys वर ताण येतो. डॉक्टरांच्या सल्ल्याशिवाय insulin किंवा tablets थांबवू नका. 4 रक्त नमुना कधी दिला याचं महत्त्व अनेकांना ठाऊक नसतं. सकाळी स्थिर अवस्थेत घेतलेले नमुने अधिक अचूक असतात. फिरून, चालून, तणावाखाली दिलेले नमुने “false high” देऊ शकतात. Testing before office hours किंवा हलक्या विश्रांतीनंतर सर्वोत्तम असते. 5 रिपोर्ट समजून न घेता औषधं adjust करणं. काही लोक रिपोर्ट बघूनच औषधं वाढवतात किंवा कमी करतात ही मोठी चूक आहे. HbA1C, fasting, post‑meal हे तीन parameters एकत्र बघूनच निर्णय घ्यावा. प्रत्येक रिपोर्ट म्हणजे doctor‑patient संवादाचं साधन, निष्कर्ष नव्हे. Lab reports म्हणजे फक्त numbers नाहीत , ते तुमच्या सवयींचं आरसाप्रमाणे प्रतिबिंब आहेत. फास्टिंग, वेळ, औषधं, आणि जेवणाचं अंतर ह्यापैकी एक गोष्ट जरी चुकीची असेल, तरी पूर्ण चित्र बदलतं. आजच ठरवा 1 टेस्ट योग्य वेळी करा 2 रिपोर्ट नीट समजून घ्या 3 औषध बदलण्याआधी डॉक्टरशी बोला तुमचं आरोग्य फक्त “numbers” मध्ये नाही , ते “discipline” मध्ये आहे! Nani Gopal, a revolutionary in Bengal in the 1902s had to flee Calcutta to avoid the British police and came to the US, where he took a job as a salesman & married an American schoolteacher His son, young Amar supplemented his family income by repairing radios. “We put up signs in all the little hardware stores where my father used to sell his imported goods,” The signs said, ‘We repair radios. So people would drop off their radios at the store and I’d take them home and repair them, and we’d give the store 10% of the invoice. I had a little pact with my father that if my grades remained good, I could go to school only four days a week and he would write an excuse saying I had a headache or something.” The teachers all knew this; it was always on a Friday and so on Monday, they’d ask me, ‘How many radios did you fix, Bose?’” Amar Gopal Bose went on to study at MIT & after he graduated and joined MIT as a professor in 1964, his mentor Y W Lee advised him to start his own electronics company. Lee said about the name “...we don’t want to be tied to a specific technology or industry because we don’t know what we’ll do in the future. The name should be pronounceable in many different languages and easy to trademark. And ideally, it’s one syllable.” Everyone laughed because they knew what he was suggesting without saying it: “Bose.” Bose started his company focussing mainly on high end products for the military. His noise cancelling headphones were originally designed for army use. Soon his audio products were making waves. Today, Bose has revenues of over $3 billion, and before he passed in 2013, Amar Gopal Bose donated a majority of his shares to his beloved MIT to help sustain their research. And it all started with one revolutionary in Bengal! Amar Gopal Bose, born on this day in 1929.
