🌟 PSMA dynamics as an early biomarker in mCRPC

A Nature Cancer study (ENZA-p substudy) shows that early PSMA upregulation (day 15) is not just a flare… it’s clinically meaningful.
🔬 Key findings:
• ↑ PSMA SUV in 68% of patients after enzalutamide
• With ↑ SUV:
– PSA-PFS 5.8 mo (enza) vs 13.1 mo (enza + ¹⁷⁷Lu-PSMA-617)
– HR 0.38 → clear benefit with radioligand therapy
• With ↓ SUV: no significant difference between arms
⚡ Interpretation:
Early PSMA increase may identify a poor-response phenotype to ARSI alone
👉 but also a targetable window for PSMA-directed therapy
💡 Take-home:
This is not progression…
👉 it’s biology revealing vulnerability

Predictive value of early PSMA upregulation for the response to enzalutamide ± 177Lu-PSMA-617 in poor-risk, metastatic, castration-resistant prostate cancer: substudy of the randomized, phase 2 ENZA-p trial

In mCRPC, PSMA expression changes early with enzalutamide. In the ENZA-p trial, 68% of patients showed increased📈 PSMA PET SUV by day 15.
This rise predicted shorter PSA progression-free survival with enzalutamide alone. However, adding 177Lu-PSMA-617 significantly improved outcomes in these patients, mitigating this adverse prognostic effect. Early PSMA SUV dynamics may serve as a predictive biomarker to guide combination therapy strategies in mCRPC. #ProstateCancer
