Isolated, periosteal stripping injuries of the Flexor Retinaculum: Case series of 3 patients with clinico-radiological review doi.org/10.1016/j.jcot… This article presents a case series of three patients (one female, two males) with isolated periosteal stripping injuries of the flexor retinaculum at the medial ankle, a rare condition often misdiagnosed. These young, active individuals (two athletes, one runner) presented with insidious onset of medial ankle pain, without a history of major trauma. Physical examination revealed localized tenderness and mild swelling over the medial malleolus, but full range of motion was preserved. There was no instability, and neurovascular status was normal. Magnetic resonance imaging (MRI) was crucial for diagnosis, revealing periosteal stripping of the flexor retinaculum from the medial malleolus, with associated mild soft tissue and osseous edema. Importantly, there were no associated fractures, deltoid ligament tears, or significant abnormalities in the tibialis posterior tendon or other surrounding structures. The authors propose a new classification system for these injuries based on MRI findings: Type 0 (normal), Type 1 (thickening), Type 2 (periosteal stripping without tendon displacement), Type 3 (stripping with tendon displacement), and Type 4 (stripping with tendon displacement and tendinopathy). The cases presented were all managed conservatively with pain relief, activity modification, and physical therapy, resulting in a full return to activity for all patients. The authors emphasize the importance of thorough clinical evaluation and high-resolution MRI in differentiating this condition from other medial ankle pathologies, such as deltoid ligament tears or tibialis posterior tendinopathy. This study contributes to the understanding of this distinct injury pattern and highlights the importance of incorporating detailed imaging and anatomical awareness for appropriate diagnosis and management of medial ankle pain, especially in active individuals. Early and appropriate management of these injuries, even conservatively, can lead to successful outcomes and a prompt return to sports or other physical activity.
