The draft GMC Order 2026 consultation just dropped. Buried in it is a fundamental change to who can be awarded a Certificate of Completion of Training (CCT). This matters enormously. Let me explain why.

The new GMC Order is undergoing consultation. The GMC is seeking to remove the speciality register and decide by themselves who should be issued a CCT. This right is currently protected in law. This is the last hurdle before the GMC will be able to issue CCTs to PAs.

I know there are many issues at present but this is the single largest threat to medicine within the UK. @gmcuk have time and again shown that they cannot be trusted. By removing the safeguards provided by Royal Colleges we will be throwing patients to the wolves.

The gravy train has arrived at its final destination. Clearly the increase in ratios for Psych and GP wasn’t the success of #choosepsychiatry as lauded by @wendyburn et. al but just the path of least resistance for IMGs with nothing to lose. No more!

However in para64, the Judges have made clear that Associates shouldn’t describe themselves as medical professionals unless they state clearly the strict limits of their role (eg work under supervision)

@Oxfordite @medicalmodelbri Incorrect Rupert. The Court of Appeal confirmed the collective noun ‘medical professionals’ encompasses doctors, PAs, and AAs See para 63 of the judgment. The litigation has been a huge own goal for the BMA.

In fact more recent meta-analyses show comparable outcomes across various settings. I've mentioned there are clear workforce planning issues for doctors. 2/

The full sentence reads: “Although nurse-physician substitution may result in better outcomes in certain cases, the evidence is uncertain.” Leaving out the uncertainty materially changes the meaning.

@parthaskar I did read it - which is why I shared it (knowing it would get coverage elsewhere). One day I am going to write an editoiral about why Drs (at least the ones on here) assume bad motives from anyone who even discussses this issues. It's very tedious.
