@resqpac LR is definitely not standard across all regions for a multitude of reasons. I'm excited to actually get some RCT level data to help clarify these risks.

In this study by Moreno et al, she evaluated trauma patients who received blood products during the first 4 hours of admission. ARDS is captured as a core quality metric. Whole blood was independently associated with ARDS. Every unit conferred a 5% increased odds of ARDS after adjusting for confounders. The reasons for this remain unclear. One unique thing about whole blood is that it is not universally leukoreduced. Unfortunately, there was no way to clearly separate ARDS from things line TRALI, which would be helpful. The real question we need to answer is WHY this is. #WholeBlood demonstrated a 99% probability of being superior to component therapy with regards to 4-hour and 24-hour mortality. Critics still waiting for that last 1% before they get started (grin).
