A few words about Shield- a new blood test for colorectal cancer screening 🌸 FDA approved Guardant Health's Shield™ blood test as a primary screening option for colorectal cancer. 🌸 Shield test meets performance requirements for Medicare coverage. 🌸 Intended for average risk individuals aged 45 or older; not for high-risk individuals. 🌸 Detects DNA fragments shed by tumor cells and precancerous growths. 🌸 It is not an equivalent alternative to colonoscopies, which are more accurate and can remove precancerous polyps. 🌸 Shield test recommended every 3 years, colonoscopy every 10 years. 🌸 False positive rate of Shield test is about 10%. 🌸 Clinical trial: Shield may miss 1 in 10 with precancerous lesions and 1 in 1,000 with cancerous lesions. 🌸 Current screening: fecal tests every 3-5 years, colonoscopy every 10 years. 🌸 Some patients resist fecal tests and colonoscopies due to inconvenience and preparation requirements. 🌸 Blood tests might lead to increased screening uptake due to convenience. 🌸 Positive Shield test results require follow-up colonoscopy. @AmCollegeGastro @CrohnsColitisFn @gastrogirl @HealioGastro @SSMHealth @SSMHealthSaints

🔴Beautiful EUS-Liver Biopsy demonstration by @helpatologist #DrBenedettoMangiavillano 👇clip With three teaching points : 1️⃣Common pitfall in EUS-LB is to be mistaken for spleen👇..... An easy way to differentiate is that the spleen will be on Extreme Clockwise rotation; observe your body position. 2️⃣for the Suction technique in EUS-LB: one, you do your pass (from puncturing the capsule [suction on] once you reach the maximum depth, close the suction and repeat the technique with each actuation; such technique will help to avoid suctioning the blood and fluid, another thing once you do your pass and you observe blood in suction Syringe such thing indicates your injured something. 3️⃣Have some time post-liver biopsy to assess the track for any immediate bleeding by Doppler. If there are signals ( blood), keep the needle 30sec. 🔴Single-session laparoscopic cholecystectomy and ERCP; Great cooperation with my colleague ⭐️ surgeon @alherz_fadhel and his outstanding efforts to adopt such an approach in a great facility. ✅️Single-session ERCP and LC are safe, effective, economically viable, and reduce hospital stay compared with performing ERCP and LC during separate sessions. Most importantly, it's linked to minimising the risk of pancreatitis as there is no manipulation with the papilla Directly with the rendez-vous approach and before that one-day procedure. ➡️I think it's deserved for multi-centre studies to assess the efficacy of such an approach and how to overcome the logistics...There are a couple of studies in the past...
