Trust CEOs, for example, are paid to oversee the collapse of services, keep quiet, and resign if necessary (with a hefty golden parachute) to deflect blame from the government.

@TJCoats None of the current major incident plans will work in the current situation. If all the ambulances are stuck outside EDs you should get a good few hours from declaring an incident until the patients arrive, it might even take long enough that the ED could be emptied.

Anyone solved the problem of major incident call out at night? In the olden days land lines and pagers worked 24/7. Now most people have only mobiles - and have them silenced at night? How does your HMIP approach this? (And has anyone done a 2am rest?)

@clare_eliza If you’ve got a half decent EPR it is trivial to do. Metrics will vary between departments as working practices vary . You need a broad range of measures, over a decent period of time and compared with peers. DM me an email address and I can send you example SQL queries

@cruncherwax If the harm is being caused by externally factors, and the organisation is united against that harm, it is manageable. When there is denial about the harm then it becomes problematic.

Does anyone in UK EM perform LP on those admitting for ?meningitis. Seems this should be our thing to do NICE: Perform the lumbar puncture before starting antibiotics, unless it is not safe to do so or it will cause a clinically significant delay to starting antibiotics.

@EM_Dr_Jacklin @DrLindaDykes If you can’t get a pan CT approved by a radiographer on an electronic request but focused CT requires tracking down a radiologist and having a discussion you may create an incentive to scan everything.

This is an interesting quandary. After massive efforts to alert EM clinicians to our failures to promptly detect injuries in silver trauma, we can’t then complain about ⬆️ CT workload 🤔 isn’t this a success rather than a problem? ⬇️
