@the_dpg @drdagly Damian I am so very sorry for your loss. Although I had not ever met him I’ve followed him for years and so deeply appreciated his advocacy. Take good care.

Because messaging is now coming out officially, I am posting to confirm that our close friend and colleague @drdagly, Darren Markland, died yesterday. As a close colleague and friend, I know that he would only want communities to come together around something like this.

You don’t need to meet someone in person in order for them to have an impact on your life. They touched so many people, more than they ever realized. We lost one of the greats today and they’ll never be forgotten. My condolences go out to their family, friends and colleagues.

@rbarbosa91 This is awesome. Thank-you. Learners frequently don't appreciate the goal of getting all the way to the LT and exposing D3/D4, especially with open vascular becoming so much less common. getting comfortable lifting up the SMA axis is like a super-power for acs

The original 1960 paper from Cattell and Braasch describing right medial visceral rotation (the Cattell-Braasch maneuver). Though most commonly associated with operations for trauma, it was originally described for tumors. The entire manuscript is 482 words long. 🧐

@rbarbosa91 Most trainees put too much tension the L hand. That distracts rather than helps tighten the knot. Gentle traction to keep control of that end is all that is necessary. You can actually just leave it loose, especially for subcuticular closure.

Note that in this first throw of an Aberdeen knot: - the L hand holds *continuous* tension on the thread - the R hand pulls on the lower strand (usually more than once) to cinch the throw down to the tissue).

🧵regarding the Aberdeen knot: a surgical knot that may be used to secure a continuous suture. It is thought to be more secure than a square knot. We will cover how to do it, what *not* to do, and also a little-known modification that may make it even more secure. (1/ )

@rbarbosa91 Definitely works, but fiddly and fumbly in the heat of the moment. Reached limit and got the Lebsche knife back onto the EDT tray. So. Much. Nicer.

I think it is likely that I would physically be able to cut across the sternum with a pair of trauma shears. (ie when doing a clamshell thoracotomy without a Lebsche knife)

6. Implement true team based care to include (and fund) nurses, PAs, NPs, social workers, dietitcians, PTs, OTs, pharmacists, etc. 7. Stop focusing on “quick fixes” that the evidence shows aren’t fixes and cost more in the long run (i.e. private clinics, two tiers). /12

This is a fantastic read. Describing lossy compression and ChatGPT gives a lot of pause for thought about the meaning of expertise, understanding and how we assess learners.⁦Reminds me of insights I’ve learned from @WalterTavanewyorker.com/tech/annals-of…

I also had to stop by Ch. Cafetería. This time, I went for the cookie and a few salted caramel chocolate truffles which I ate without taking a picture of but you should know they were outstanding. Even if you don’t like chocolate, you need must visit them on @stonyplainroad.

@karimbrohi No data, yet, but anxiety disorders seem to be extremely prevalent in this group, admitted to our trauma service. Both for MVC and violence. ?maladaptive response to anxiety/stress leading to these decisions

Wondering if the risk taking behaviours of young men in their cars have the same underlying causes (and overlap with) their propensity to interpersonal violence.

They’re not on twitter but if anybody knows staff from the Royal Alex Hospital 🏥 please extend my most sincere thanks to each and every one of them for the care I’m receiving. If you’re going to break five ribs this is the place you want to be. So ‘thanks guys!’ First class.
