Histologic grade is still the best predictor of outcomes in PXA and we don't yet know the significance of MVP or necrosis in PXA.

I've dealt with cases that looked exactly like GBM in the past, had long-term survival, and then reclassify as PXA based on genomics. In those cases I'd argue it's easiest to consider them PXA.

There is really very limited published data to suggest how a tumor like this will behave.

It's the right location, it's well-circumscribed, it has BRAF V600E, and lacks any GBM or ependymoma molecular alterations. It scored a perfect 1.0 on the methylation classifier.

No one has done the study to show that tumors matching to methylation class PXA that are high grade and don't look like PXA behave like PXA.

@thepathphd I'm a neuropathologist, but interested in brain tumors rather than dementia. Clinical and research training in pathology give you a different skill set and are very different career paths.
