New in @JAMAOnc: Our study of >11,000 PLCO participants shows that ~25% of elevated PSA values fall below threshold within a year, but persistent elevations rarely do (<10%), and should proceed directly to further eval. @SigridCarlsson @MariaPereMD @UrologyMSK @VickersBiostats

How fast do #rcc tumor thrombi grow? 📈 In our cohort @MayoUrology (n=141), venous tumor thrombi expanded ~1 cm/month (wide range). Faster growth seen with higher Mayo level, sarcomatoid/rhabdoid features & metastasis. These data can guide pre-op imaging & surgical triage.

@VickersBiostats @Moh_AhmedMD “Relatively minor harms” is relative. Why expose a patient to any additional harm? Always a balance of risks/benefits. Oversimplified to argue case closed here and current selection criteria is the best we can ever do

@Moh_AhmedMD At MSKCC we use the rule "if you aren't high enough risk for PLND, you aren't high risk enough for RP". Given the very major harms of RP (urinary, erectile dysfunction) vs. relatively minor harms of ePLND, why is "patient selection" such an issue?

When an RCT and an observational study have virtually identical results, you can be a little more definitive: ePLND offers zero biochemical recurrence benefits but does reduce metastasis
