A patient with Duchenne MD: society values his health state at 42/100. He rates it 65/100. Whose number should guide his ICD decision? We surveyed 156 cardiac NMD patients to see what self-perceived health actually tracks. 🧵

In Friedreich ataxia, self-rated health exceeded societal valuation by 29 points. In Duchenne, by 22. The gap reverses in myotonic dystrophy type 2, where EQ-5D misses fatigue, apathy, cognition. A bidirectional gap argues against measurement artifact.

In NMD, cardiac death is now the leading mode of mortality. Pacemakers, ICDs, and CRT are increasingly on the table. These decisions hinge on quality of life. And QOL is usually inferred from how disabled the patient looks. That's the failure mode.
