It is not opinion. It is basic math. About 29k U.S. MD and DO graduates compete for roughly 44k residency positions each year. That leaves 13k to 15k seats that must be filled by someone qualified. That is why the U.S. recruits top physicians from around the world, trains them to its standards, and integrates them into its workforce. They do not merely fill gaps. They sustain patient care, drive research, and strengthen the entire system. This is part of why U.S. medical training remains among the strongest in the world to recruit international talent and maintain high standards. The system is also tightly regulated and deliberately gatekept through strict residency requirements. Even fully trained physicians from abroad must repeat 3 to 5 years of U.S. residency, reinforcing consistent standards while inherently limiting direct global competition. If the goal is to increase the number of U.S. citizens entering medicine, then address the barriers within the pipeline. Expand financial support for medical education. Improve resident compensation and working conditions. Do not scapegoat the physicians who sustain the system. That approach does not lead to reform. It leads to failure and harm.

I need to talk about this. I moved to US 4.5 years ago to go to Kellogg. Got my MBA and now I work at a top consulting firm averaging 75+ hours a week helping some of the biggest American companies grow. That is literally all I do and think about - how to make these businesses better. And I'm not unique in this. There are thousands of Iranians in this country doing exactly the same. Engineers, doctors, researchers, founders. People who came here the right way, jumped through every hoop, and just work. Really, really hard. But this USCIS hold has frozen everything in their lives. Green cards, H-1Bs, OPT for students graduating in weeks... even spouses of American citizens can't get their cases processed. People who've been here for years and did nothing wrong. And the part that kills me is I'm hearing from really talented friends that they are seriously looking at leaving for Canada or UK. These are people America invested in and should be fighting to keep, and instead we're pushing them out the door. How does that make any sense? Then the government, or whoever made that policy, should refund his green card and visa renewal fees, return the $150,000 sign-on bonus tied to the job he can no longer keep, and reimburse his employer for the sudden loss of a surgeon from a program they were building. You cannot take people’s money, induce reliance, let them build their lives around a legal process, and then abruptly declare it meaningless. That looks a lot like fraud. I do not know how constitutional law would frame it, but someone should also be asking whether damages are owed for the harm caused to patients whose care is disrupted in the middle of follow-up. And if patients suffer complications because they lost their surgeon mid-course, who is responsible for that harm? The government? The hospital? Or no one?

H Res 965 is backdoor amnesty for hundreds of thousands of Haitians who came into our country illegally. It is an abject disgrace and a shameful betrayal of everything Republicans ran on in 2024. All conservatives should vehemently rejected it. Unfortunately, the bill was just discharged on the House floor with 219 votes. We will vote on final passage of the bill tomorrow.

Vice President JD Vance stated that new H-1B visas are down roughly 90% under the Trump administration through administrative action alone. To clarify: H-1B visas are temporary work visas for specialty occupations, with an annual cap of 85,000 new visas (65,000 regular + 20,000 for advanced-degree holders). Vance was referring specifically to new issuances (initial approvals for first-time entrants), not extensions or renewals for workers already in the U.S.

Had a fun day today. 2 weeks ago an Indian (US Citizen) IT employee called ICE on his own department because he heard that many of them were here on fake H1-B Visas. 25 employees were arrested out of 100ish. He was correct. After talking to his leadership he also informed them that many here legally,but we're hired under false pretenses, because they had bought diplomas in India and never really got a degree. At this point management was worried they had been defrauded so they called me up to help them figure it out. We ran all of the remaining employees hiring paperwork through some AI analytics today. Massive amounts of documents. This included reading copies of diplomas, and and a ton of other personal documents. Training the AI analytics tool to look for irregularities in past work experience and education history. As an example: 1.) Check to see if the school exists 2.) Check to see if the person who signed the diploma actually worked at the university 3.) check to see if the degree listed is actually taught at that university 4.) ETC ETC ETC. We found that of the remaining 75 H1-b employees 50 of them had provided fake diplomas to either our company directly, or the contracting company they were hired through. So out of this one department of 250 people, 100 ish were from India here on visas, and 75 of them had defrauded the company during the hiring process. Either providing fake visa paperwork or falsification of education. Basically 75% lied. Not sure what they are going to do about it yet, as across the IT organization they have 1500 ish H1-B visa employees and they cannot fire 75% of them right now even if they wanted to, but I do know they are running all current visa employees records through the tool, any new applicants as well, and will be firing anyone who lied as soon as they get replacement in line for them. From the leadership I was working with: "We have to remove them, we cannot keep employees who defraud the company even if they are good and cheap"
