@r_aravinth_ Perhaps the relevant question is whether the chances of saving kidneys are significantly better than chances of ischemia. I doubt the trials included v. heavy proteinuria. Many excluded PAD. Perhaps the horse is out of the barn.

#askrenal 70/M with Diabetes-Severly increased albuminuria.Normal renal function. Has asymptomatic upper extremity PAD and Claudication in lower limbs.Started on DAPA by Physician.Should we stop it?

PPI's in CKD: 3 pts in CKD clinic with PPIs restarted since I held them previously (usu adding an H2B) Indications: GERD (by GI), cough/COPD (by Pulm). None for Barret's or GI erosion. Frustrated my colleagues in other specialties and I are working against ea. other. Thoughts?

If people wonder why I keep switching patients from short-acting furosemide to longer-acting torsemide, it’s because of the top graph. Green bar above dashed line (na excretion from furosemide) rapidly approaches pink area (compensatory reabsorption)—>0 net naturiesis.

@MaryPrante @NEJM Assuming steady-state, intake = excretion. Generally regarded as a better estimate than, say, food questionnaires or food diaries. Though not as precise as a laboratory-supplied diet, it allows long-term evaluation.

Anybody have any experience with RenaMag? It is a magnesium supplement with 150 mg of elemental magnesium. I’m a bit concerned about the sticker to correct the amount of magnesium from 126 to 150 mg.
