@r_aravinth_ Perhaps the relevant question is whether the chances of saving kidneys are significantly better than chances of ischemia. I doubt the trials included v. heavy proteinuria. Many excluded PAD. Perhaps the horse is out of the barn.

If people wonder why I keep switching patients from short-acting furosemide to longer-acting torsemide, it’s because of the top graph.

@MaryPrante @NEJM Assuming steady-state, intake = excretion. Generally regarded as a better estimate than, say, food questionnaires or food diaries. Though not as precise as a laboratory-supplied diet, it allows long-term evaluation.
