The patient with persistent atypical atrial flutter after PVI. CCW activation around mitral annulus. LIPV reconnected and the gap can be seen on its anterior aspect. Anterior mitral line slowed and terminated the flutter. Re-isolation of left TVs and CTI line afterwards.

Young PFA is already as effective as RF - a technology refined over 30 years!

My explanation was possible epicardial connection via Septo-Pulmonary bundle and endocardial breakthrough at right inferior part of the posterior LA. Ablated on the roof where ablation slowed and terminated the tachycardia.

39 yo F s/p aortic and mitral valve replacement and cryo PVI. Redo case with RSPV reconnection and re-isolation. Induced MRAT with CL of 424 ms. Confusing caudo-cranial activation on both posterior and anterior LA. Good PPI at proximal CS and LA roof.

#EPeeps Ablation of cavo-tricuspid isthmus dependent atrial flutter using a focal monopolar pulsed-field ablation catheter: Feasibility, periprocedural coronary spasms and conduction disorders. ✴️84 patients from 2 large EP centers… 👉🏻For focal #PFA vs #RFA radiofrequency: - the proportion of first pass block was higher (93% vs 55%) - procedure times were shorter (7 [5,11] min vs 17 [15,19] min) But ‼️Despite nitroglycerine administration: - 4 patients (5%) presented transient ST elevation - 2 patients (2%) showed a transient complete AV block during ablation. Conclusion: ✏️ CTI ablation using focal PFA is feasible, but operators should be aware of rare but critical conduction disturbances and coronary spasm during the procedure, even when preventive measures such as nitroglycerine administration are employed.
