I am hearing multiple practices are now cancelling appointments to make way for the Wednesday rules. So rather than offering appts, GPs will now be spending their time picking needles out of haystacks. What an absolute waste of time. Access instead of Continuity. Sigh.

Cancelling GP F2F appts so they can process all the requests that come in online - because the contract now demands online access open all of core hours This is a worse service to pts who need GPs most Opening the doors on a full bus service doesn’t create more seats - more GPs

@Neal94G @Parody_RCGP Total triage itself can be a lottery. Some will find patients overload it with simple queries because they can. Some will have nurses triage some have GP which means less time them seeing patients (in both cases) Online triage platforms also come at a cost themselves.

@Parody_RCGP Thanks Parody 🙏 they’re fine. No is also fine, but a model of “no space today, try UTC or try again tomorrow” in lieu of some form of triage (like total triage systems) seems bad. And also feel sorry for reception team who no doubt get a lot of abuse for “refusing appointment”

Relative contacted GP practice for non-urgent issue. Reply saying “we are now at full capacity” and to attend UTC if symptoms are “urgent for you today”. On asking if could have appointment in next few days, told no and to go to UTC. How is that acceptable?

Too many doctors I represent are facing unemployment in 8 weeks time. Following our recent emergency @BMANorthernRDC meeting, I have written to every CMO/ MD and DME in our region. We’re calling on trusts to protect & support northern’s doctors through August’s crisis.

Hundreds of resident doctors across our region are facing unemployment. Not enough specialty training posts & huge NHS cuts are leaving doctors without jobs in August. As a temporary measure, we have called on all trusts to support & protect doctors in our region through this.

@DrEilidhMaria I’ve had 2 coils now took paracetamol and ibuprofen beforehand was fine. These were however after the birth of my children about a year later both times. 100% rate them though.

20 doesn’t seem like very many in a 12h shift, TBH. I imagine the slow points come from the technicalities of delivering care rather than the actual direct clinical contact though. That is one plus of GP - there’s not a lot we can get so our winter lists of 100+ patients is quicker to assess, plus we’ve got space. I wonder what might make things quicker in ED. Is over-investigation a thing? Are full clerkings done, or focused histories?

If you’re an ACP seeing 20 patients on a ED night shift, you probably do need to consider whether you’re doing a good job and who for. If ED just wants to be a ‘is this medics or surgeons’ service it should just not be a specialty anymore. Embarrassing.

This sums up the Kafkaesque binfire that is the current NHS: my son was referred to @AlderHey SALT 2 years ago. We still hadn't had an appointment so his teacher chased the referral up. Today we discovered the referral has been rejected for being out of date. Make it make sense?!

This is not the first time I have seen doctors on #MedReddit suggest the idea of deliberately sabotaging their own appraisal in order to remain employed. @wesstreeting this is absolutely shameful. Medical training in the UK is completely broken.

I'm a mother - an involved and loving one at that - and I have no idea what my kids' shoe sizes are. I just take them to Clarks twice a year and buy what the lady tells me to 🙃 Do other parents all know their kids' shoe sizes off by heart?!
