I also like work horse wires for getting back into the true lumen because the tactile feedback is better than jacketed wires and if is easier to know when the tip is in the wrong plane. You can do something even crazier by using an undersized cutting balloon and create fenestration to re-enter.

60s male ACS. No prior Cath. Plaque dissection in mid RCA. Initial wiring kept going to false lumen. Unable to IVUS .So PTCA prox RCA with 2.0🎈. No Twinpass in lab. IVUSed site of entry into false lumen. Under live IVUS used whisper to enter true lumen. Completed PCI. Pt went 🏠

It’s all about implementing optimal technique. Not all operators perform bifurcation stenting well... doing the math: an average operator (by volume) performs 10-15 bifurcations per year.... go figure...

That’s why #RadialFirst always and always! Balloon oclusion + US/fluoro guided thrombin injection for large femoral hematoma w/ clinical repercussion after a femoral PCI. Tip is to dilute some contrast on thrombin solution, locate needle position w/ US and inject during fluoro

I dissected one early out of training during a PCI w/o obvious dampening. Ever since I have almost universally advanced a guide extension a cm or two into the LIMA as long as the vessel is good sized. I haven’t dissected again🙏

After an LAD PCI via LIMA had a good discussion w our advanced interv fellows. Good discussion for FITs in general. 1 MUST constantly be aware of pressure wave form 2 Must know where guide tip is at all times 3 begin with soft, 0.5 gram, WH wire

@lucaguirre31 My algorithm has always been no protamine until the equipment is out of the coronaries. I’ve seen cases where a perf gets compounded by vessel thrombus which makes a bad situation worse😱 Leave the ACT therapeutic and give protamine at the end if you must

I guess the lesson is find somebody to sleep with who knows how to do CPR. Seriously don’t waste your time with non-CPR people, it’s a matter of life or death.
