#renalpath 70F w high globulin count + severe AKI, Renal Bx. #electronmicroscopy shows abundant cylindrical structures in tubular casts. By EM, it looks like cryofibrinogen. A glom is unremarkable. My understanding is cryofibrinogen is typically found in glom loops. Any thoughts?

Can anyone identify these crystals? 80M, native kidney bx for investigation of paraprotein. Patient has had a gout flare. I suspect the material is uric acid but would like confirmation. I think the material has been intraluminal and now surrounded by injured epithelial cells.

Lost another patient today because we don't have enough staff to do dialysis in Seattle hospitals. As doctors we're asked to "prioritize" daily i.e. guess who might live without treatment. Simple solution: pay nursing staff a living wage and get rid of for profit healthcare.

Lemons to lemonade: 67 yo M w/ HTN and 1.7 g protein. Secondary FSGS, next case? NOPE. Rare gloms for LM; huh, silver negative stuff in mesangium? NO gloms for IF, cut deeper, found ONE. IgG-kappa. Congo red positive. ONE sclerosed glom for EM, used it. Amyloid, the biopsy ninja.
